Field Operations Policy Mo <br />1. The Company representative completes the Initial Condition Analysis Form (FIN- <br />CS106), signs and forwards the form to the funeral home or funeral director in charge. <br />a. The decedent check-in process in Carepoint is an acceptable alternative to the Initial <br />Condition Analysis Form (FIN-CS106)for those PCCs implemented on Carepoint. <br />2. Refer to Personal Effects for information related to verifying personal effects. <br />For after -hour removals by a third party contractor: <br />1. The third party contractor delivers the decedent to a restricted, designated room or area <br />(i.e. refrigeration or preparation room), away from and out of public view, ensuring the <br />decedent is covered with a sheet or gown. <br />2. The third party contractor secures and logs valuables in a designated locked, secure area <br />and completes the Safe Log (FIN -CS 107). <br />3. A Company representative completes the decedent Initial Condition Analysis Form (FIN - <br />CS 106) (see above) upon arrival the following morning. <br />4 Personal Effects <br />4.1 Storage and Handling of Personal Effects <br />Policy. Anything that comes with, is worn by or is provided for the decedent (with the <br />exception of hospital garments) must be considered a personal effect, and the Company must <br />inventory and secure each item. For cremation cases, the Authorized Representative must <br />provide and/or give permission for any personal effect(s) to be cremated with the decedent or <br />placed in the final container with the cremated remains. <br />All valuables must be secured in a designated locked, secure area that is out of public view or <br />access and which is used only for securing decedent personal effects. Valuables delivered to the <br />funeral home must remain in the designated locked, secure area until it is time for placement on <br />the decedent unless specifically required for preparation or ship -out. <br />Procedure. <br />When taking custody of personal effects at the time of decedent removal: <br />If there are no personal effects, check the IF NO PERSONAL EFFECTS box on the <br />Personal Effects Inventory Form (FIN-CS006) and skip to procedure 4. <br />2. Prior to the decedent's removal where family is present, have the family remove and <br />retain all jewelry and other valuable items from the decedent if possible. <br />a.
