Field Operations Policy Procedures <br />section of the Personal Effects Inventory Form (FIN-CS006). <br />3. The Company representative signs as "Receiving Associate" in the Acknowledgement at <br />Receipt section of the Personal Effects Inventory Form (FIN-CS006). <br />4. Place any valuable or breakable items (i.e. jewelry, glasses, money) in a clear, sealable <br />bag with a label showing the decedent's full legal name (LAST, First Middle), date of <br />birth, date of death, funeral home and case ID number (if applicable). <br />5. Store the bag(s) containing the valuables in a designated locked, secure area with a copy <br />of the Personal Effects Inventory Form (FIN-CS006). <br />6. Log bags placed in the designated locked, secure area on the Safe Log (FIN-CS106). <br />7. Store non -valuable items (i.e. clothing, shoes) in a designated area with a copy of the <br />Personal Effects Inventory Form (FIN-CS006). <br />Keep valuables received at the funeral home and only transport valuables from the <br />funeral home as it relates to final disposition of those items. <br />9. The funeral home communicates the Authorized Representative's requested disposition <br />of items remaining in the custody of the PCC (i.e. bury or cremated with decedent, <br />return to family). <br />Example: When the Authorized Representative requests that an item be cremated with <br />the decedent, the funeral home transfers the item to the crematory or PCC to be placed <br />with the decedent for final disposition. <br />Transporting personal effects from one location to another: <br />1. Ensure all items to be transported are present by verifying them against the Personal <br />Effects Inventory Form (FIN-CS006). <br />2. If applicable, log the bags out of the designated locked, secure area on the Safe Loi <br />(FIN -CS 106). <br />3. When transferring personal effects from one location to another, the transferring location <br />keeps a copy of the Personal Effects Inventory Form (FIN-CS006) and sends the original <br />along with those items to the receiving location. <br />Note: If the funeral home is transferring personal effects to the PCC or crematory, the <br />original Personal Effects Inventory Form (FIN-CS006) remains in the funeral home file. <br />4. Secure personal effects in the transport vehicle for transfer. <br />5.
