Case ID number, if available <br />iii. Date of birth <br />iv. Date of death <br />V. Location number (from which decedent came) <br />vi. Date and time checked in <br />vii. Name of Company representative checking in the decedents <br />viii. The space number in the refrigeration unit where the decedents are placed, using <br />the first cell of "Space Nbr." on the log for the initial placement. <br />ix. If decedents must be moved from one space to another within the refrigeration <br />unit, write the subsequent space number in the second cell of "Space Nbr." on <br />the same line. <br />4. When checking decedents out of the refrigeration unit, record the below information in <br />the Checked Out section of the Refrigeration Overflow Loi (FIN-CS086). <br />a. Date and time checked out <br />b. Name of Company representative checking out the decedents <br />c. Purpose for check out from refrigeration unit <br />5.
