All required forms must be completed and signed by the appropriate individuals <br />(Company representatives and/or the Authorized Representative). <br />Note: See Appendix A — Required Forms for information on specific forms used in this <br />document. <br />Procedure. <br />1. Retain original copies of forms at the funeral home in the decedent's case file. <br />2. Give copies of forms to the Authorized Representative if he/she signed those forms. <br />2.3 Form Modifications <br />Policy. Modifications or variations to any form format must be submitted to and approved by <br />the Operations Support and the Legal Departments before they are used. <br />Procedure. <br />1. Contact the Operations Support Department (FieldOpsSupportgsci-us.com) for <br />requested modifications. <br />2.4 Decedent Name on Forms and Identification Band <br />Policy. The full legal name of the decedent must be used on all forms and the white <br />identification band (see White Identification Band).
