Field Operations Policy and Procedures <br /> 2.5 Changes to Incorrect/Incomplete White Identification Band <br /> Policy. Incorrect/incomplete information on a decedent's white identification band must be <br /> corrected and may involve replacing the white identification band to ensure all information is <br /> legible with no cross-outs, correction fluid or write-overs. Two Company representatives must be <br /> present when correcting or replacing and attaching a white identification band to a decedent. <br /> Information such as the middle initial or middle name may be written on the white identification <br /> band as long as it can be added legibly. <br /> Procedure. As soon as it is determined that any information on the white identification band is <br /> incorrect or incomplete: <br /> 1. The Company representative who identifies the discrepancy immediately stops the <br /> process and researches the inconsistencies. This may include contacting the funeral <br /> home, removal service, next-of-kin or place of death to verify the identification and <br /> information of the decedent. <br /> 2. Once the discrepancies have been thoroughly researched and resolved, the Company <br /> representative fills out the Incorrect/Incomplete Identification Band Form (FIN-CS005) <br /> noting the confirmed accurate information. <br /> 3. Two Company representatives correct the existing white identification band (or replaces <br /> and attach a new white identification band) with the confirmed accurate information (see <br /> White Identification Band) and sign the Incorrect/Incomplete Identification Band Form <br /> (FIN-CS005). <br /> 4.
