City of Stockton Draft Amended Report of Waste Discharge(AROWD) Austin Road Landfill Work Plan ■ The first alphanumeric set will represent the well location:The shallow aquifer well at the MW- A location will be identified as MW-15S and its sample code will be MW15S;and the deep aquifer well will be identified as MW-15D and its code will be MW15D. This alphanumeric set will also be followed by a hyphen. ■ The next two letters followed by a hyphen will represent the media type;GW for groundwater. ■ Finally,a three-digit code will be used to establish the sampling event: 001 for the first event, 002 for the second event,and so forth. For example,the code MW15S-GW-001 would indicate a groundwater sample(GW) collected from monitoring well MW-15S during the first sampling event(001). The actual sample identification code will be documented in the field log book. 2.2.4.4 Sample Labeling Sample labels will contain an abbreviated summary of the information contained in the field log book. Sample label information will be written with waterproof,indelible ink. Sample labels will contain the following information: ■ job name and number ■ Sampling site ■ Sample identification code ■ Date and time of sampling ■ Signature of sample collector 2.2.4.5 Sample Handling and Chain-of-Custody It is necessary to provide documentation of the handling of each sample from the time it is collected to the time it is discarded or archived. To maintain a record of sample collection,transfer between personnel,shipment,and receipt by the laboratory,documentation in log books and a chain-of- custody record will be employed. Chain-of-custody sheets consist of multi-part carbonless forms that list the I.D.numbers of the samples in custody,time and date of sample collection,sample type,analytical parameters, and a series of signatures documenting each time the samples are relinquished or received. Typically,a chain-of-custody sheet documents the transfer of a group of samples shipped together. The original custody record is shipped along with the samples,while the initiator of the record retains a copy. When charge of a specific group of samples changes several times,each custodian may not retain a copy of the custody record. This is acceptable as long as the original custody record indicates that each person accepting the samples has subsequently relinquished custody appropriately. Chain-of- custody forms will be completed according to the following protocol: ■ The originator fills in all requested information from the sample tags. ■ The originator signs in the top left"Relinquished by"box and keeps the copy. ■ The original record sheet is shipped with the samples.
