DAILY FIELD REPORT Page 1 of 1 <br />Project Name: Foothill Sanitary Landfill - Area 2A Report Number: 045 Page 2 <br />Project Number: 14-101-01 Date: Monday 07/13/15 <br />Owner: Dept. of Public Works, San Joaquin County CQA Monitors: Jude <br />Contractor: R J Gordon Construction, Inc. M. Hu and <br />Weather: Clear Wind: 0-10 mph. <br />Temperature: Approx. range 70- 92 F Other: <br />At approximately 8: 30 am the construction of the test pad requested by Geosyntec began. The contractor chose to build the <br />pad on the foundation floor in the SW corner of the floor. A CAT 14H motorgrader began by brushing off the surface which <br />was dry, (due to sitting over the weekend.) One laborer picked 3 rocks from the surface of the subgrade. The surface was then <br />rolled and cross rolled using a CAT CS 54B smooth drum roller. The liner contractor then set out GSE GCL panels to cover an <br />area of approximately 20 ft by 50 ft. The GSE GCL was not a roll from this job, but had similar properties and was acceptable <br />per the procedures memo. A 50 foot long by 22.5 wide piece of GSE HDPE DST from this job was then placed over the GCL. <br />A CAT D6 LGP dozer was then used to push the adjacent stockpile of 5/16 LCRS Gravel Drainage Material from Cemex on to <br />the liner as a normal operation would do. The lift was spread in a 9" layer per plans and specs, checked by GPS and also with a <br />probe. The material was tracked 20 times using the CAT D6 dozer. <br />Up to 2 laborers and various pieces of equipment were used to remove the Gravel from the HDPE liner, including shoveling by <br />hand, the Cat 14H blade and the CAT 320 excavator. After the entire sheet of liner was cleaned off, the Geosyntec engineer <br />performed a visual examination of the liner and cut a sample for archive. The operation was over at 2 noon. No perforations <br />or other damage was noted. <br />Geosyntec representative Krzysztof Jesionek was present and in charge of the operation. The test pad was also observed by <br />the County including Taj, Joyesh, Awni and others. The activity was also observed by AES QA Monitor M Huggard. <br />Equipment Onsite: (1) CAT D8T dozer, (1) CAT D6T LGP dozer, (1) CAT D6R dozer (with slope board) (1) CAT 621G water <br />pull, (4) CAT 627 scrapers, (1) 14H motorgrader, (1) CAT 825H compactor, (1) water truck, (1) Hitachi 300 excavator, (1) <br />CAT CS 54B smooth drum roller (vibratory type), (1) CAT TH160B folk lift, (1) CAT side -boom winch smooth drum roller, (1) <br />CAT 430D backhoe.
