health; and any other items of concern. The Operations Manager records these inspections on a form that Forward has prepared specifically for this purpose. A copy of this form is included in Appendix M. Forward Landfill retains these forms for at least three years from the date of each inspection. The Forward Landfill Contingency Plan is included in Appendix J. The plan describes the actions Forward personnel will take to respond to a release of ACW. In addition, prior to any excavations, or other activities that may disturb buried ACW, Forward will review the records of ACW disposal to determine if the proposed activities are likely to disturb buried ACW. The most likely activity that may disturb buried ACW is installation-of landfill gas (LFG) collection wells, or trenches. If any disturbance is likely, or possible, Forward will prepare an excavation management plan that meets the requirements of 14CCR 17897.21(b). If the proposed disturbance is installation of LFG collection facilities, the plans and specifications for the installation will be submitted as a separate, stand alone document as set forth in 14CCR 17897.21. Forward maintains asbestos disposal manifests and operating records. An example of the operation record is included in Appendix M. The operation record includes the quantity and date of each ACW shipment, the disposal location, a summary report of any incidents which require implementing the contingency plan, results of inspection required by 14 CCR 17898.19 (b)(2)(B) and training records as required by 14 CCR 17897.19 (c)(2)(B). The notices required by 22 CCR 66268.7 (a)(11) (A-B) are retained at Forward's sales office in Stockton, along with a copy of the Hazardous Waste Manifest. All hazardous waste(ACW) manifests are retained onsite for five-years. Forward will make these records available to the LEA upon request. Forward meets the existing requirements of 27CCR for financial responsibility for closure, postclosure, and operating liability. Forward provides certificates documenting compliance wit these requirements as requested by the LEA and CIWMB. 6.4.8.4 ASH DISPOSAL METHODS The Forward Landfill accepts ash for disposal.
