Andrew Keller ® - 2 - 11 February 2008 <br />a. The Discharger shall include a Construction Quality Assurance Plan (CQAP) for <br />earthwork inspections and repairs to the low hydraulic conductivity layer, the <br />vegetative layer, and the vegetation of the landfill cover. Prior to the initiation of <br />any maintenance construction work, the Discharger shall receive concurrence <br />from the Regional Water Board on the CQAP. In addition, a third party who is <br />independent of both the Discharger and the construction contractor shall oversee <br />the performance of all of the maintenance construction, quality assurance, <br />monitoring, and testing. <br />b. The Discharger shall include a work plan to repair the erosion, rills, gullies, and <br />vegetation. <br />c. The Discharger shall include documentation that maintenance personnel have <br />been properly trained in the identification of problem areas and the procedure for <br />repairs to problem areas. Training shall include but not be limited to stormwater <br />drainage control and management to prevent/mitigate rills and gullies; vegetation <br />replanting, irrigation, and germination; and all stormwater best management <br />practices. <br />d. The Discharger shall include a schedule with dates for the initiation and <br />completion of the repair and mitigation measures. <br />3. By 30 July 2008, the Discharger shall submit a report documenting the cover repairs; <br />vegetation replanting, irrigation and germination; and implementation of improved <br />stormwater best management practices. <br />The site inspection report and photographs are attached.
