sb <br /> proper site management and control of the work area by the Project Superintendent. <br /> Since slips, trips and falls constitute one of the most common safety hazards on <br /> construction related sites, workers can prevent injuries by looking out for the following <br /> potential hazards: <br /> - Uneven terrain; <br /> Precariously positioned objects, such as broken glass, sharp metal objects or <br /> construction related equipment; <br /> Wearing personal protective eQuipment; <br /> 4- Use of heavy equipment; <br /> 2.4.4 Underground and Overhead Utilities <br /> Reasonable efforts will be made to identify the location(s) of underground utilities (e.g., <br /> pipes, electrical conductors, fuel lines, and water and sewer lines) before mechanized <br /> soil intrusive work is performed. The state underground utility notification authority <br /> (e.g., USA, Dig Alert, Blue Stake) will be contacted prior to the start of intrusive field <br /> activities in accordance with local notification requirements. In areas not evaluated by <br /> the underground utility notification authority, and where a reasonable potential for <br /> underground utilities exists, one or more of the following techniques will be employed <br /> to determine the location of subsurface structures: <br /> _L contracting the services of a Qualified private utility locator <br /> - conducting a survey of the subject area by staff trained in the use of subsurface <br /> utility locating equipment <br /> subsurface testing (i.e., potholing) to the expected depth of probable utilities <br /> (not less than 5 feet) <br /> If utilities cannot be located or if unlocated utilities are suspected to be present, <br /> subsurface activities (i.e., borings, excavation) should not be conducted prior to <br /> confirming the location(s) or absence of underground utilities. In addition, equipment <br /> with articulated upright booms or masts shall not be permitted to pass within 20 feet of <br /> an overhead utility line (less than 50 kV) while the boom is in the upright position. <br /> 2.4.5 Personal Protective Equipment <br /> Some safety hazards are a function of the work itself. Personal protective equipment <br /> (PPE} can impair a worker's agility, hearing and vision, which can result in an increased <br /> risk of an accident. Site personnel should constantly look out for potential safety <br /> hazards, and should immediately inform the SSO of any new hazards so that mitigative <br /> action can be taken.
