sb <br /> 11 Medical history including a record of pre-existing conditions; <br /> 21 Occupational history, including a record of pre-existing exposures; <br /> 31 Physical exam, including blood pressure, pulse, blood count, etc.; <br /> 4] Pulmonary Function Tests, including FVC and FEVL <br /> The physician should note the following conditions when evaluating employees: <br /> I] Determination of fitness to work while wearing protective eQuipment; <br /> 4- All persons entering the exclusion zone shall be able to don Level B. <br /> 21 Conditions that could increase susceptibility to heat stroke, [i.e., obesity and <br /> lack of physical exercise, etc.]; <br /> 31 Conditions that could affect respirator use, such as missing or arthritic fingers, <br /> facial scars, dentures, poor eyesight, or perforated ear drums. <br /> 4.3.2 Periodic Medical Examinations <br /> For those personnel working on these and other similar remedial action projects, the <br /> following medical tests should be given on a periodic basis. The time span between <br /> periodic evaluations is based upon exposure monitoring results and the physician's <br /> recommendations. <br /> 11 Update of medical and occupational history; <br /> 2] Physical examination; <br /> 3] Additional testing based on the following: <br /> - examination results <br /> - exposures <br /> - job classification and task <br /> 41 More freQuent testing based on specific or new exposures. <br /> 4.3.3 Exit Examinations <br /> Exit examinations are offered to each employee already part of a medical surveillance <br /> program. They are conducted to document an employee's health status at the <br /> conclusion of site remedial activities or termination of employment with the company. <br /> Although this type of examination may be viewed as a means of limiting employer <br /> liability, this use is only appropriate for non-latent health effects.
