Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />March 14, 2019 <br /> Facility Address: <br /> 1131 Holly Dr, Tracy <br /> Facility Name: <br /> Certified Tire & Service Center Tracy <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 114 HSC 25160.2(b)(4) Consolidated manifest receipts failed to contain complete information. <br />The consolidated manifest receipts for 2017, 2018, and 2019 Used Oil were missing the permitted recycling facility. <br />The service work order #148799 was missing the date and generator signature. All copies of each receipt shall <br />contain all of the following information: <br /> - The name, address, identification number, contact person, and telephone number of the generator, and the <br />signature of the generator or the generator's representative; <br /> - The date of the shipment; <br /> - The manifest number; <br /> - The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br />type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br />Nations/North America (UN/NA) identification number, if applicable; <br /> - The name, address, and identification number of the authorized facility to which the hazardous waste will be <br />transported; <br /> - The transporter's name, address, and identification number; <br /> - The driver's signature; <br /> - A statement, signed by the generator, certifying that the generator has established a program to reduce the <br />volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br />economically practicable. <br />Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br />all required information. <br />This is a minor violation. <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br />equipment. <br />The black and yellow spill pallets located in the hazardous waste storage room contained liquid and sludge. All <br />facility communications or alarm systems, fire protection equipment, spill control equipment, and decontamination <br />equipment, where required, shall be tested and maintained as necessary to assure its proper operation in time of <br />emergency. Immediately begin this practice and provide evidence to the EHD. <br />This is a minor violation. <br /> 601 CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days. <br />-One black metal 55 gallon drum of paper filters was observed in the hazardous waste storage area with an <br />accumulation start date of 6/28/2018. <br />-One blue plastic 55 gallon drum of antifreeze was observed in the hazardous waste storage area with an <br />accumulation date of 3/16/18. <br />-One blue plastic 55 gallon drum of antifreeze was observed in the hazardous waste storage area without an <br />accumulation date. <br />Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste <br />stored on site at any time may store waste on site up to 180 days.
