1..� N%.4, A return visit was conducted on July 15, 2011 to confirm excavation of the distribution box installed under SR0057183. The distribution box had been located and excavated. Per, "River Rats"Pumping Company, there was an obstruction of"blackened handy-wipes"that had been blocking the flow from the main distribution box to the new seepage pits installed under SR#0057183. Flow was observed and confirmed at the new system's distribution box this date by the EHD. No further action by the END is warranted. Personnel of"River Rat"Pumping Company were instructed to contact Tom McCoy to apprise him of the findings. Ensure that MHP tenants are kept out of the WWTP area for their safety and welfare. 8. The plant is currently being evaluated by personnel of Quality Service to determine the functionality of the WWTP sewage disposal fields. The WWTP and its components are maintained and operated on a routine contractual basis by Mr. Tom McCoy, OMIT, of Quality Service, Inc. Continue to maintain and operate the plant efficiently. A copy of this report will be sent to La Cumbre Management Company and Tom McCoy. For the permitting year 2011-2412, analytical testing and reporting to the EHD is required as part of a condition of the operating permit. The wastewater shall be required to be sampled for the following constituents by an employee of a state certified analytical laboratory and results submitted to the EHD: TKN- Included Nitrates and Ammonia BOD- Biological Oxygen Demand TSS - Total Suspended Solids pH CBOD- Carbonaceous Biological Oxygen Demand A wastewater treatment plant operator, or certified operator certified by the State of California Water Resources Control Board, shall be available to operate the plant. The EHD currently lists Mr. Tom McCoy on file as the certified operator. A copy of a current certification shall be submitted to the EHD and maintained in the permanent file. A copy of the certificate is not on file. Submit a copy to the EHD upon receipt of this inspection report. The EHD currently lists Segal Family Trust DTD as the legal responsible party.
