BEM Project No. 11-9234 <br /> Visual Observation <br /> A full visual assessment of the subject site was conducted to determine the severity of contamination <br /> inside the house,metal barn,and(1)modular trailer. <br /> Condition of the Residence <br /> Upon arrival of the residence it was noted that the required CHD notification signage was posted on the <br /> exterior of the front entry door. <br /> General: The personal contents within the house, metal barn, and (1) modular trailer were removed, <br /> which raises the concern of cross contamination to the next location receiving the contents. The exterior <br /> grounds of the subject site show no signs of illegal dumping. <br /> Kitehen/Dining Area: The kitchen area is in poor condition. All of the appliances (refrigerator, <br /> microwave,range/oven unit)were removed.The cabinets show no signs of staining related to the spillage <br /> of chemicals.The kitchen counter top and sink drain show no signs of staining or clumping. <br /> Bat11r-00111s: The bathroom flooring materials and the vanity surfaces show no signs of spillage. The <br /> sink chains and bath tub drains show no signs of clumping into the septic waste water system. <br /> Living Room: This area is in good condition. The carpeting was in tail-condition and no evidence of <br /> spillage was noted. <br /> Bedr-00111S: The bedrooms are in good condition.The carpeting was in fair condition and no evidence <br /> of spillage was noted. <br /> Metal Barn: No personal items were noted within the barn. The barn consists of metal construction <br /> with a concrete floor. There is a sink cabinet located in the North/West corner of the barn. No signs of <br /> chemical spillage or dumping were noted within the barn. <br /> Modular Trailer: Several windows are broken out and interior is in poor condition. No personal <br /> items were noted inside the trailer. No signs of chemical spillage or clumping were noted within the <br /> trailer. <br /> P.O.
