■ Summary of the expected chemical,physical, and biological hazards anticipated at the job site; <br /> ■ Brief listing and location of the onsite safety equipment to be used as necessary; <br /> ■ Level of protection required under this Site Specific Health and Safety Plan and what included items; <br /> ■ Location of nearest phone and emergency numbers; <br /> ■ Inform all personnel that smoking, eating, or drinking is not allowed within the exclusion zone; <br /> ■ Name, location, and brief directions to nearest hospital or clinic to be used in case of emergency <br /> (Reference The Hospital Location Map); <br /> ■ Site-specific hazards worker should be aware of including traffic hazards,observers,trip hazards,traffic, <br /> etc.; <br /> ■ Inform attendees of the location the health and safety plan if needed during the work day; and, <br /> ■ After meeting is complete please have all attendees sign the On-Site Safety Meeting Attendees Sign-off <br /> Sheet in Appendix B, documenting they have attended the meeting, and understand all necessary items <br /> and procedures listed in this health and safety plan. <br /> 3.5 PERSONAL DECONTAMINATION PROCEDURES <br /> Decontamination is the process of removing or neutralizing contaminants that have accumulated on personnel <br /> and equipment. Hands and exposed skin should be cleaned using soap and water after leaving exclusion zone <br /> and prior to eating or drinking. Used gloves, sampling materials, and respirator cartridges should be <br /> contained in a plastic or paper sack to be discarded in a trash receptacle. Clothing worn on the job site <br /> should be removed as soon as possible and washed with soap and water.
