ACTION GUIDE#10— EXPLOSION/VESSEL RUPTURE <br /> GENERAL INFORMATION: <br /> • A Vessel explosion or rupture is a very rare event. Despite its rarity, this event is possible <br /> in LAWFT facility. <br /> • It is very hard to anticipate when a vessel could rupture. However, fire is one situation <br /> where a vessel rupture is more likely. <br /> • PRVs are designed to prevent vessel overpressure but like all pieces of mechanical <br /> equipment, occasionally they do not function properly. <br /> INDICATION: <br /> • Increasing pressure in process systems. <br /> • Fire. <br /> • PRVs malfunctioning. <br /> IF QUALIFIED,THE FIRST RESPONDER SHOULD FOLLOW THIS SEQUENCE: <br /> 1. Call 9-1-1, as appropriate. <br /> 2. Sound alarm. Alert others in the area. <br /> 3. Without entering vapor cloud or confined area, stop source. <br /> 4. If possible by remote ESD -Stop any sources of ignition within the area. <br /> 5. Assess situation, move upwind and outside of danger area. <br /> 6. Don't smoke. <br /> 7. Deny access. <br /> 8.
