• All individuals relinquishing and receiving samples shall sign, date, and indicate the time <br /> in the lower portion of the COC. <br /> If an error is made on the COC prior to custody transfer, it shall be crossed out and initialed, and <br /> the correct information written above the error. If an error is made after custody transfer, the <br /> sampling personnel shall immediately notify the laboratory and any other organization to which <br /> the information was submitted of the correction. <br /> The COC shall accompany the samples at all times while the samples are in transit. Upon <br /> receipt of the samples at the laboratory, the laboratory sample custodian shall sign and date the <br /> COC, indicating acceptance of the samples. The original (white copy) of the completed COC <br /> shall be maintained at the laboratory until submittal of analytical results, at which time the <br /> original, or a photocopy if other laboratories are involved, is remitted with the analytical results. <br /> A copy of the completed COC shall be maintained by the project. <br /> 4.2 Field Notes/Notebook <br /> A bound field notebook shall be maintained by the sampling team to provide daily records of <br /> significant events, observations, and measurements during field activities. All entries will be <br /> signed and dated. All members of the sampling team shall use this notebook, which will be kept <br /> as a permanent record. <br /> Entries into the field notebook will be made using indelible ink. Each page of the field notebook <br /> shall be sequentially numbered at the top and dated. Each page shall also be signed and dated <br /> at the bottom by the person responsible for the entry, and any blank space at the bottom of the <br /> page shall be marked out to indicate that it has been left blank intentionally. Corrections will be <br /> made by striking out the incorrect information with a single horizontal line, such that the <br /> incorrect information is still legible, and the correct information will be entered above the strike <br /> out. All corrections will be initialed and dated. <br /> It should be noted that field notebook entries should be factual, detailed, and objective. The <br /> field notebook shall be scanned and attached to the final report as an addendum. The following <br /> are items which should, if applicable, be recorded in the field notebook during field activities: <br /> • Name and location of Site; <br /> • Name and affiliation of sampling team members <br /> • Time of arrival on Site; <br /> • Weather conditions; <br /> • Arrival and departure times of visitors to the sampling location; <br /> • Health and safety-related issues (Site Specific HSP and JSAs should be onsite at all <br /> times); <br /> • Summary of equipment preparation procedures; <br /> • Summary of day (e.g., drilling time, down time, etc.); <br /> • Dates and times of sample collection or event; <br /> • Numbers and types of samples taken, sample location, and sample identification; <br /> • A description of sampling methodology; <br /> • Field measurements of samples; <br /> • Record of telephone calls and/or contact with people at the Site; and <br /> • Any problems or unusual observations. <br /> Used Oil Sampling and Analysis Plan 6 July 29, 2014 <br /> BNSF Railway Multiple Locations <br /> TRC Project No. 211983 <br />
