Site Description and History <br /> o Describe ownership, operator and parent company history in- <br /> cluding names, addresses, telephone numbers and dates. <br /> o Has this company, or any other related business entity, had <br /> any location other than this one. If so, what are the ad- <br /> dresses. <br /> o What is the size of this facility ( acres) and have there been <br /> any changes in the property boundries. <br /> o What is the number, size (sq. ft. ) and type of buildings on <br /> the site. <br /> o Are there any site plans, maps and/or aerial photographs of <br /> the site available for review. <br /> III. Site Activity - Historical and Current <br /> o Describe all products manufactured and specific types of site <br /> activity. _ <br /> o Describe all processes and the steps therein. <br /> o Describe any changes over time in the processes ( i .e., sub- <br /> stitution of chemicals, equipment replacement, etc.) and/or <br /> products manfactured. <br /> IV. Hazardous Materials <br /> o Describe the types of hazardous materials ( i .e. , toxic, cor- <br /> rosive, volatile, radioactive, persistent) used as raw <br /> materials, catalysts, cleaning agents, etc. for each product, <br /> process and maintenance operation. Ask to rev1e— Material <br /> Safety Data Sheets. <br /> o What are the physical states ( i .e. , solid, powder, liquid, <br /> gas, etc. ) of each hazardous material used. <br /> o What are the estimated annual quantities of each hazardous <br /> material used and changes in use over time. <br /> o What are the names of all suppliers of these hazardous <br /> materials. <br />
