SAN J O A Q I I I N Environmental Health Department <br /> L -0 U N T Y I�I AMENDED <br /> 10/28/2019 <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree Distribution Center-Stockton 1 1122 Runway Dr, Stockton October 14, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> least one of the hand soap bottles beneath the conveyor belt was missing its pump dispenser(IMG 8822). <br /> 35) Found on the MOD floor grate,two of three bottles of Assured Hand Soap with leaky caps and nearly empty <br /> (IMG 8825-6). <br /> 36) Found on the MOD floor grate, at least one of eight bottles of green Ajaz liquid dish soap, open with a broken <br /> cap and seemingly empty(IMG 8827-8). <br /> 37) Found on the MOD floor grate, at least three of 17 fallen containers of Floral Gel Beads were damaged (IMG <br /> 8829-30). <br /> 38) Found on the MOD floor grate, at least one of the 11 fallen bottles of light blue hand soap was open, nearly <br /> empty, missing its cap, and had historical leakage around its cap rim (IMG 8847-9). <br /> 39) Found on the MOD floor grate, a damaged cardboard box contaminated with leaky bottles of Baby Oil (IMG <br /> 8851-2). <br /> 40) Found on the MOD floor grate, at least two of the(at least)eight fallen bottles of Klar&Danver blue hand <br /> soap were seemingly empty(IMG 8854-5). <br /> 41) Found on the MOD floor grate, a box containing several green V05 Men 3-in-1 Shampoo/Conditioner/Body <br /> Wash, one of which was open and had signs of past leakage on its cap(IMG 8856-7).A metal beam next to the <br /> box of shampoo had green staining that appeared to be the V05 shampoo(IMG 8858). <br /> 42) Found on the MOD floor grate, a damaged leaky bottle of Kar&Danver Body Wash, missing its pump <br /> dispenser(IMG 8861-2). <br /> 43) Found on a wooden pallet in the aisle designated "Hazardous Product Storage Area,"three bottles of glass <br /> cleaner, one of which had a broken spray nozzle, and another of which appeared empty(IMG 8864-5). <br /> 44)Six open unlabeled containers of dusty chemical containers and trash were observed on the north side of the <br /> facility(IMG 8866, 8871-2). Five of the containers were nearly filled with containers of an orange Ajax cleaning <br /> solution and what appeared to be trash, such as used paper towels and broken wood pieces(IMG 8867-8). Four of <br /> the five containers with the Ajax had signs stating"Do not throw away!Waiting for repack boxes"(IMG 8867-8). <br /> One of the six containers was partially filled with trash, such as used paper towels, plastic bags,dirt,and damaged <br /> cardboard,and with the following chemical items(IMG 8869-70): <br /> -At least two loose bottles of Klar&Danver Body Wash, and a damaged cardboard box that appeared to be <br /> filled with more bottles of the body wash. <br /> -At least six tubes of Jot Tempera Paint <br /> -At three bottles of Assured Tussin Mucus&Chest Conjestion medicine <br /> -At least one bottle of Arm& Hammer 3-in-1 Body Wash/Shampoo/Conditioner <br /> -At least one bottle of pink nail polish <br /> -An open package containing two AA Alkaline Sunbeam batteries <br /> 45)On the north side of the facility, several unlabeled closed cardboard boxes were observed stacked two rows <br /> high on a pallet beneath several silver 16 fl oz bottles of SMB Synthetic Blend 1OW-30 SAE Motor Oil (IMG 8872, <br /> 8878).At least seven of the cardboard boxes were seen with dark staining/leakage(IMG 8872). <br /> 46)On the north side of the facility, several unlabeled closed cardboard boxes were observed stacked four rows <br /> high on a pallet(IMG 8873).At least 15 of the cardboard boxes were seen with dark staining/leakage(IMG 8873-4, <br /> 8879-81).The boxes contained silver16 fl oz bottles of SMB Synthetic Blend 1OW-30 SAE Motor Oil (IMG 8875-7). <br /> 47)On the north side of the facility, an open cardboard box of several boxes of Sensitive Fluoride Anticavity <br /> Whitening Toothpaste, at least four of which were damaged, and a light bulb,was observed on a cart(IMG 8882). <br /> 48)A 96 fl oz container of LA's Totally Awesome Bleach was found damaged and missing its cap, and a solid air <br /> freshener was found missing its label, inside an open unlabeled black tote near the hazardous waste storage area <br /> (IMG 8926-8).There were several bottles of Fabuloso multi-purpose cleaner,Ajax liquid dish soap, Palmolive liquid <br /> dish soap, and Air Wick aerosol cans also found inside the black tote. <br /> FA0013602 PR0539024 SCO01 10/14/2019 <br /> EHD 22-03 Rev.9/20/2019 Page 29 of 48 RCRA Large Quantity Hazardous Waste Generator OIR <br /> 1868 E.
