COMPLAINANT <br /> PICKED UP ORDER AND BEGAN EATING IT WHEN THEY ARRIVED HOME. COMPLAINANT ALLEGES THEY BEGAN <br /> TO CHOKE ON A BAY LEAF.THEY THEN THREW UP. COMPLAINANT'S SON FOUND 2 BAY LEAVES IN THEIR FOOD <br /> AS WELL. COMPLAINANT REPORTED IT TO EMPLOYEES AND THEY ALLEGEDLY MADE A JOKE OUT OF IT." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Manager on duty is aware of the complaint. Per Manager,the bay leaves are only added to the rice(white or brown rice). They <br /> are used for seasoning the rice. Per Manager,the rice is rinsed (approximately three times).After rinsing the rice,the rice is <br /> added to the rice cooker with water, oil and bay leaves(3-6 bay leaves depending on the size of the batch of rice). The rice is <br /> cooked,then more oil is added on top of the rice and the bay leaves are removed. <br /> Upon inspection this date, no bay leaves were observed in the white rice or in the brown rice. <br /> Remind employees to continue to be mindful of removing bay leaves from the rice. <br /> No signature obtained. <br /> Report typed up at the office 3:14pm-3:26pm <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: Discussed w/Briant Jimenez, Svc.
