Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CO4GFOXEK Site Location: 1740 HOUSTON AVE <br /> Activity Summary <br /> Activity Date Recorded by <br /> 02/27/2020 Called Carolyn who stated Clean Harbors was on site at 8am and estimated the FLORIDO <br /> clean-up would take them about 3 hours. Carolyn confirmed she would call the ER <br /> cell phone when EHD should go out to verify the clean-up. Notified Carolyn that <br /> Jason Gaitan, EHS, will be the inspector to verify the clean-up. <br /> Received a call back from Carolyn stating that Darryl with Clean-Harbors was <br /> called to another emergency, so the clean-up will happen either this afternoon or <br /> tomorrow. Carolyn to text the ER cell phone Darryl's contact info and notify EHD <br /> when the clean-up will be performed. <br /> 02/27/2020 Was updated for response by ER person. It was determined that I would not be ALANIZ <br /> responding to the cleanup at night. <br /> 02/27/2020 Discussed complaint with Elianna Florido (RENS) and John Alaniz (RENS). GAITAN <br /> Received and reviewed information from Elianna. <br /> 02/27/2020 Received notification from Carolyn, property owner. Clean Harbors will begin GAITAN <br /> cleaning up spill around 2pm. Updated John Alaniz (REHS). <br /> 02/27/2020 Received contact information for Eduardo, supervisor for Clean Harbors. GAITAN <br /> Contacted Eduardo-(530) 723-3262. He stated that he is on site and that eta for <br /> clean up to be finished is approximately 3 hours. Requested he contact ER phone <br /> an hour in advance to allow EHD to view finished clean up. Updated John Alaniz <br /> (RENS). <br /> 02/27/2020 Discussed clean up with John Alaniz (RENS) after discussion I contacted Carolyn GAITAN <br /> Godwin, city of Stockton. <br /> She agreed to verify clean up and contact EHD if clean up is not satisfactory. She <br /> stated that she will be sending photos of site clean up. <br /> Sent text to Eduardo Macias, supervisor for clean harbors. Updating him that EHD <br /> will not be on site and not to update us on when site clean up will be finished. <br /> 02/28/2020 Received photos from Carolyn Godwin, City of Stockton of site clean-up. Carolyn GAITAN <br /> states Ramos oil will be picking up the waste today at 1 pm. She states that the <br /> floor was wet and muddy due to Clean Harbors due pressure washing and <br /> scrubbing and scraping the waste. Confirmed that I will meet her out there. <br /> Discussed photos with Elena Manzo (Lead Senior REHS).Generated photo log <br /> from photos. <br /> 02/28/2020 Met with Carolyn Godwin, City of Stockton. Reviewed clean up. Carolyn states that GAITAN <br /> Clean Harbors pressure washed and scrubbed and scrapped fertilizer spill. <br /> Carolyn states that fertilizer trail found in front of maintenance shop were swept <br /> up. Three 55 gallon drums with debris with fertilizer were generated and kept on <br /> site. Carolyn states drums will remain on site until scheduled clean-up next week <br /> of maintenance shop and trailer with fertilizer next week. Ramos oil was on site to <br /> remove the four 55 gallon drums with oil, one empty 55 gallon drum, and contents <br /> within one oil tank. Requested a copy of disposal receipt from Carolyn once <br /> Ramos Oil finished hazardous waste pick up. <br /> Updated Elena Manzo (Lead Senior REHS). Generated photo log. <br /> 02/28/2020 Received photos from Carolyn Godwyn, City of Stockton and message that she GAITAN <br /> will be scanning over manifest information.
