Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CO4GFOXEK Site Location: 1740 HOUSTON AVE <br /> Activity Summary <br /> Activity Date Recorded by <br /> 03/11/2020 Received an email from Carolyn indicating Clean Harbors in on-site today to FLORIDO <br /> continue the clean-up. Carolyn stated she would let me know what time the <br /> clean-up will be completed. I responded to Carolyn's email, notifying her that an <br /> EHD inspector will verify the clean-up once completed. <br /> Received the following email from Caroly: "Eduardo thinks it should be about 6-7 <br /> hours. He reminded me that they need to discover and manifest the chemicals <br /> found in the container before removing. We had several arsons on site, one <br /> before the big clean up and one last night. They burned a few things, so the <br /> containers may not match up from your original list to the removal list. Do you <br /> want to wait for the removal before you inspect?" <br /> I called Carolyn to clarify her email. Carolyn stated Clean Harbors estimates they <br /> will be done with the clean-up of the conex box around 4 pm today. Clean Harbors <br /> will place waste found in the conex box in temporary boxes, clean the conex box, <br /> place the boxed waste and the clean-up waste back into the conex box and lock it. <br /> Safety Kleen will pick-up and dispose of the waste, including the waste drums (at <br /> least three) generated from the clean-up of the maintenance building. Carolyn also <br /> stated that there have been two arsons, resulting in the empty crushed metal <br /> drum that was inside the maintenance building to be burned, as well as possibly <br /> some of the chemical containers (observed at the time of my inspection)to be <br /> burned. <br /> Carolyn states that the waste drums from the maintenance building clean-up will <br /> stay inside the locked bathroom until Clean Harbors returns to pick-up all of the <br /> waste for disposal. <br /> I told Carolyn to let me know when the clean-up is nearing completion today so <br /> that EHD may go out to verify. If EHD does not verify the clean-up today before <br /> 5pm, Carolyn stated she can also show the EHD the inside of the conex box <br /> tomorrow morning. <br /> Called and spoke to Eduardo of Clean Harbors, who provided the following <br /> information: <br /> -Three 55-gallon drums of(solid?)waste were generated from the fertilizer spill <br /> clean-up. <br /> -Three 55-gallon drums of solid waste debris was generated from the clean-up of <br /> the maintenance building floor(broken flourescent bulbs, dirt, mice/rat feces). <br /> -Two 55-gallon drums of liquid waste was generated from pressure washing the <br /> maintenance building floor. <br /> -The chemical containers, found inside the maintenance building at the time of my <br /> inspection, will be placed in either the locked bathroom or the locked conex box. <br /> -One 55-gallon drum of liquid waste was generated from triple washing the <br /> red-dye diesel tank that will be sent to a recycler by Clean Harbors. <br /> -Various brands of waste fertilizer and buckets of unknown material found in the <br /> conex box will be separated and placed in tri-wall boxes. The conex box will be <br /> cleaned, and the waste generated including the fertilizers and unknown material <br /> will be locked in the conex box. <br /> -Safety Kleen will be profiling and disposing of all of the waste. <br /> Eduardo to inform me when he is getting close the completion of the clean-up so <br /> that EHD may go out to verify. Emailed Carolyn letting her know that since Clean <br /> Harbors is not scheduled to go back out to the site after today, EHD will have to <br /> verify the clean-up of the maintenance building and the conex box today so she <br /> should let me know an hour before the clean-up is complete. <br /> 5105.rpt Page 5 of 8 <br />
