Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CO4GFOXEK Site Location: 1740 HOUSTON AVE <br /> Activity Summary <br /> Activity Date Recorded by <br /> 03/26/2020 Reviewed an email received from Carolyn on 3/26/20, which contained a draft FLORIDO <br /> letter responding to the violations documented on the Notice To Abate, as well as <br /> photos, service orders, waste profiles, and the generator copy of the manifest for <br /> the 4 drums of waste gasoline. <br /> The letter also confirmed that the 500 gallon convault tank that previously held <br /> gasoline, was removed from the site on 3/9/20 and will be stored, until a new use <br /> is determined, on City of Stockton property, specifically at Public Works Fleet <br /> Maintenance Corp Yard at 1465 S Lincoln St in Stockton, according to emails <br /> received from Carolyn on 3/4/20 and 3/9/20. Carolyn states in her 3/4/20 email <br /> that Fleet Maintenance hired a vendor to remove the tank, and Chad Reed, Fleet <br /> Manager, (209-937-7143) can answer questions regarding the tank. <br /> Carolyn emailed me the following: "Did I miss any key issues in this draft letter? <br /> Stephen will review and sign this afternoon after a conference call. We will submit <br /> tomorrow if you don't have any recommendations." <br /> I emailed Carolyn the following: " I reviewed the draft documentation you emailed <br /> me. Do you know how the broken fluorescent lamps and the microwave, in the <br /> maintenance building, were managed and how they will be disposed of (as <br /> hazardous or non-hazardous wastes) ?Also, one recommendation I have is this: <br /> once you obtain the final disposal records for all of the wastes, please ensure <br /> each waste/spill noted on the Notice To Abate is accounted for on the disposal <br /> records. <br /> Do you have a date for when Safety Kleen will be picking up the wastes for <br /> disposal?" <br /> Carolyn responded with: "The contents of the maintenance building were <br /> containerized as hazardous waste-in drums. We did not remove anything from <br /> the site. <br /> He moved the empty paint cans into the locked area, but did not containerize <br /> them for hazardous waste. He will dispose of those accordingly. <br /> I will make sure the manifests are complete. <br /> Patrick has not returned my email. I will reach out to Eduardo for the final <br /> removal." <br /> Emailed Carolyn back requesting she let me know the date of disposal once she <br /> knows it. <br /> 03/31/2020 Received an email from Carolyn on 3/30/19 stating Clean Harbors is scheduled to FLORIDO <br /> remove the waste from the site today. Emailed Carolyn requesting she provide a <br /> copy of the waste disposal records she receives today. <br /> Received an update from Carolyn stating that the microwave that I saw during my <br /> initial inspection was burned in the fire that was caused by arson at the property. <br /> 04/07/2020 Reviewed narrative. FLORIDO <br /> 04/07/2020 Reviewed clean-up waste generator manifests received from Carol on 3/31/20. FLORIDO <br /> Parts of the manifest are illegible, but appear to account for all of the waste <br /> observed by Lydia Baker, REHS, on 3/11/20. Emailed Carolyn reminding her to <br /> provide the a copy of the manifests once they are signed by the designated facility, <br /> and to provide a receipt of recycling for the diesel tank. <br /> 5105.rpt Page 7 of 8 <br />
