Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CO0050265 Site Location: 1000 ROTH RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 08/07/2019 Spoke with Michael Algots, Hazardous Materials Management, Union Pacific, via ALANIZ <br /> phone regarding the brown liquid. Two containers were stacked on top of each <br /> other. Brown liquid was observed on the side of the bottom container so it was <br /> believed that the top container was leaking. Michael Algots hired Patriot <br /> Environmental Services (Patriot) to contain the release and clean it up. Patriot <br /> searched through the suspected leaking container(top one) and did not find any <br /> evidence of a discharge. It is currently unknown what the brown liquid on the <br /> container is and where it came from. It is suspected that there was a previous <br /> unreported release. Michael Algots had Patriot clean up the liquid on the side of <br /> the bottom container and generated one 55 gallon drum of waste. The waste is <br /> going to be profiled by GHD and then be properly disposed of. <br /> 10/01/2019 Sent an email to Michael J. Algots ALANIZ <br /> <mjalgots@up.com>, asking for an update. <br /> 06/04/2020 Received disposal records from Derek Smith, Patriot Environmental, ALANIZ <br /> <dsmith@patriotenvironmental.com>, on 6/4/2020. Sent a reply back asking for <br /> waste determination documents. <br /> 06/05/2020 Received a call from Paul Holt, 562-490-7020. He said he would have the ALANIZ <br /> analytical sent to me. <br /> 07/10/2020 Sent an email to Derek Smith <dsmith@patriotenvironmental.com> asking for the ALANIZ <br /> waste profile. <br /> 10/12/2020 Reviewed complaint and still need to received the analytical results. Sent an email ALANIZ <br /> to Michael J. Algots <mjalgots@up.com> and Paul Holt<pholt@up.com> asking <br /> for the analytical results. <br /> Received the analytical results from Michael Algots. <br /> Gathered the complaint documents together and abated the complaint.
