Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CO0052167 Site Location: EAST BOUND HWY 26 EAST OF FINE RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 07/30/2020 Downloaded inspection photos. Generated photo log. FLORIDO <br /> 08/03/2020 Discussed gasoline spill with Muniappa Naidu, CUPA PC, and Elena Manzo, Ld Sr FLORIDO <br /> REHS. It was determined that the gasoline spill had not been dug up and that I will <br /> contact the construction company to let me know if they plan on digging the spill <br /> area, and to put the contractor and the clean-up company in communication with <br /> each other so that they can coordinate a day for the clean-up contractor to perform <br /> the clean-up without the construction company on-site performing their work. <br /> 08/03/2020 Site inspection to locate gasoline spill, located the spill, took pictures, emailed MANZO <br /> pictures to Elianna for further follow up along with the contractor's contact <br /> information. <br /> 08/05/2020 Reviewed photos of gasoline spill location provided to me by Elena Manzo, Ld Sr FLORIDO <br /> REHS. Called and left a voice message for Steve Rubat(209-352-2351), overseer <br /> of the construction (Hwy 26 widening) going on near the spill location. According to <br /> Elena, who visited the site, the gasoline spill appears to be located immediately <br /> north of and adjacent to the temporary signal light for the traffic going west on Hwy <br /> 26. 1 will request Steve Rubat to block off the area where the gasoline spill is <br /> located, in order to preserve the spill in that location until the clean-up can be <br /> performed. I will inform Steve that I will put him in contact with All Environmental <br /> Services so they can coordinate a day for the clean-up to be performed. <br /> 08/05/2020 Reviewed narrative. FLORIDO <br /> 08/11/2020 Called and left another voice message for Steve Rubat, since he hasn't returned FLORIDO <br /> my call. Emailed Juan Armendarez and Brandon Espinoza the following: <br /> "Regarding the diesel and gasoline spills on Hwy 26, 1 left a voice message last <br /> week and today for Steve Rubat(209-352-2351), the construction contractor for <br /> the Hwy 26 widening, but haven't heard back from him. I was hoping to put you in <br /> contact with Steve, to see if you guys could coordinate a day to finish the <br /> clean-ups. Note that as of 8/3/20, the approximate area of the gasoline spill <br /> located on the north side of Hwy 26, was still intact, but had been driven over with <br /> the construction equipment(see gasoline spill area in the attached photo).The <br /> gasoline spill area was marked for the USA and still needs to be excavated. <br /> Would it be possible for someone to go out there with a PID monitor to locate the <br /> exact location of the gasoline spill and caution tape it?Also, please let me know if <br /> you're able to contact Steve and coordinate a day to conduct the clean-ups." <br /> 08/12/2020 1 received an email from Barbara Butler(CHP) at BButler@chp.ca.gov. She just MURO <br /> returned from vacation and was requesting status on the cleanup. <br /> notified Elianna Florido(EHD) of the status request. She briefed me on the <br /> status. <br /> I replied to Barbara Butler and let her know the site is still pending cleanup by All <br /> Environmental Services; they are waiting on current Cal Trans road construction. <br /> The road construction has no tentative date for it's completion. <br /> 08/20/2020 Sent Elena Manzo, Ld Sr REHS, an update on this complaint. Spoke to Elena, FLORIDO <br /> who stated we will give the clean-up contractor a chance to complete the clean-up <br /> after the construction on the highway is complete. <br /> 5105.rpt Page 6 of 8 <br />
