Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COJFGLFE5 Site Location: 6540 S AUSTIN RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 10/12/2020 Received an email from Mark Zachary stating Rick Nelson will be able to help me FLORIDO <br /> with my request. Emailed Mark and Rick, letting Rick know to contact me if he has <br /> any questions about the information I am requesting. <br /> 10/12/2020 Generated photo log of photos provided to me by Russ Cole. FLORIDO <br /> 10/12/2020 Called Patriot Environmental and spoke to Ash Moghaddam, Richmond ER FLORIDO <br /> Manager, who referred me to Derek Smith, Sacramento ER Manager. Derek <br /> Smith stated he will have Brandon Eck call me. Received a call from Brandon Eck <br /> who provided photos of the spill, clean-up, waste, and generator copy of the <br /> hazardous waste manifest. According to Brandon, 1 pint of cueva 2.4 copper soap <br /> fungicide spilled onto the trailer floor due to a puncture near the top of the tote; the <br /> spill clean-up generated one drum of non-RCRA haz waste absorbent& debris; <br /> waste transported for disposal using uniform manifest 019703745JJK <br /> (10/10/2020); and the punctured tote was sealed and left on-site. Brandon stated <br /> he believed the tote was going to be sent back to Estes Express. <br /> 10/19/2020 Reviewed and revised narrative. FLORIDO <br /> 10/19/2020 Received a voice message from Bob of Geo Analytical requesting a call back from FLORIDO <br /> me since he is assisting Estes in responding to the Notice To Abate. Bob states he <br /> is not aware if any waste was generated from the clean-up. Called and left a voice <br /> message for Bob notifying him that one drum of haz waste was generated and that <br /> I still need to know what happened to the punctured tote that still had soap <br /> fungicide inside of it. <br /> 10/27/2020 Reviewed final signed manifest 019703745JJK for the 100 pounds of non RCRA FLORIDO <br /> hazardous waste absorbent& debris contaminated with soap, received from <br /> Robert Marty, President of Advanced Geo, Inc. The final signed manifest crossed <br /> out the word "soap" and replaced it with "oil."The material that spilled and was <br /> cleaned up was Cueva 2.4 Copper soap fungicide, according to the clean up <br /> contractor, Patriot. <br /> 10/28/2020 Reviewed Notice To Abate. Emailed Robert Marty back requesting he confirm if it FLORIDO <br /> was Cueva 2.4 Copper Soap that spilled, since the word "soap"was replaced with <br /> "oil" on the manifest, and if so to provide a safety data sheet for the soap. Also <br /> requested him to explain how the punctured tote of soap was managed. <br /> 11/16/2020 Called and spoke to Rick Nelson, Estes West Regional Safety Manager, who FLORIDO <br /> states the tote involved in the spill incident was returned back to the shipper. Rick <br /> also states he does not know why the waste disposal manifest states the <br /> absorbent and debris is contaminated with "oil" instead of soap. Rick states it was <br /> soap that spilled. Rick emailed me a statement confirming that the tote was <br /> returned to the shipper. <br /> 11/16/2020 Reviewed narrative. Organized complaint documentation according to new FLORIDO <br /> complaint abatement procedures. Submitted complaint documentation to Elena <br /> Manzo, Ld Sr REHS, for abatement. <br /> 11/19/2020 Printed photos of the spill cleanup, received on 10/12/20 from Brandon Eck of FLORIDO <br /> Patriot Environmental. Added photos to complaint documentation. <br /> 01/06/2021 Reviewed and completed complaint documentation revisions from Elena Manzo, FLORIDO <br /> Ld Sr REHS. Submitted complaint for scanning. <br /> 5105.rpt Page 2 of 2 <br />
