Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COUEZKMF6 Site Location: 11530 W. EIGHT MILE RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 12/11/2019 Met with Sherri Keys, Manager for King Island Marina and Charles Spurlock, GAITAN <br /> property owner. Charles stated that he contacted Parker Diving Services to <br /> conduct the clean up. <br /> CJ Spurlock, son of the property owner, took me to the 37-foot sunken vessel <br /> located in the Empire Dock Space 11. 1 observed booms and absorbent pads <br /> surrounding the sunken vessel. I noticed oil actively being released from the <br /> sunken vessel. I walked to the neighboring spaces and observed an oily sheen in <br /> all the spaces (spaces 1-14). 1 requested if CJ could place more booms and <br /> absorbent pads to help better contain the release. CJ and his employees grabbed <br /> more booms and placed them in the surrounding area. <br /> Contacted Muniappa Naidu (Program Coordinator) updated him on spill. Received <br /> a call from Elena Manzo (Lead Senior REHS) she requested I contact fish and <br /> wildlife and coast guard and let them know that the vessel is actively leaking oil. <br /> She provided the information for Mitchell Goode and Matthew Plaza, Wardens for <br /> fish and wildlife. She also requested if the property owner could give me an <br /> estimate on how much oil is in the sunken vessel. <br /> Talked to Charles Spurlock, he stated that the vessel had no diesel in the engine. <br /> He explained that the vessel had been sitting there for some time now. He stated <br /> that only oil he can think of comes from the two engines and a generator that is on <br /> the boat. He gave an approximation of 5 gallons. <br /> Contacted Mitchell Goode, Warden for fish and wildlife. Left a voicemail. <br /> Contacted Matthew Plaza, Warden for Fish and wildlife. Let Matthew know about <br /> the situation, he stated that he had been notified of the sunken vessel. He asked if <br /> I could check on the sunken vessel to see if it still actively leaking. I went to <br /> Empire Dock space 11; 1 stated that it did not appear to be actively leaking <br /> anymore. He stated that fish and wildlife would not be able to arrive on site. <br /> Received call from Jason Raymond, Coast Guard he let me know that he will not <br /> be coming on site. He mentioned that he is in contact with the property owner, <br /> Charles Spurlock. He said that the Coast Guard would come on site if the property <br /> owner cannot find a third party to clean-up the sunken vessel. <br /> Contacted Elena updated her on responses from fish and wildlife and coast guard. <br /> Generated notice to abate and went over it with Charles Spurlock. He signed the <br /> notice to abate; I sent a copy to his email. <br /> Sherri Keys let me know that Parker Diving Services will not be able to perform the <br /> clean up and that they are trying to contact Vessel Assist to do the clean up now. I <br /> asked if she could notify me if Vessel Assist agrees to do the clean up. She stated <br /> that she would. <br /> 12/11/2019 Spoke to Francisco Ruiz (office assistant) regarding complaint number generated. GAITAN <br /> He stated that Willy Ng (Operations), had generated his own complaint. Discussed <br /> with Willy he let me know to refer CO0051329 to COUEZKMF6. <br /> 12/11/2019 Received call from Sherri Keys, Manager for King Island Marina. GAITAN <br /> She notified me that Vessel Assist will be doing the clean-up tomorrow(12-12-19). <br /> 12/11/2019 Updated Elena Manzo (Lead Senior REHS), Lydia Baker(RENS), Claudia Muro GAITAN <br /> (EHS Trainee) about the spill. <br /> 12/11/2019 Generated photo log and typed up narrative.
