EIGHT MILE RD Activity Summary Activity Date Recorded by 12/12/2019 Received update from Elena Manzo (Lead Senior REHS) regarding clean up of GAITAN sunken vessel in King Island Marina. Based on update I contacted Mathew Plaza, Fish and Wildlife Services. (FWS) He requested a text message of the property owners contact information and clean-up progress. Contacted Jason Raymond, Coast Guard.. He was out of the office, I relayed the message to front desk staff, requested he call ER Phone. Updated Elena. Contacted Lydia Baker(REHS) let her know that I contacted fish and wildlife and coast guard. I gave her the owner's contact information. I asked if she could send that to Mathew Plaza, warden for FWS. I asked if she can text him about how Vessel Assist were managing the clean up. 12/13/2019 Completed daily for yesterday. Completed photo log. Made copies and gave to BAKER Jason Gaitan (EHS) for the file. 12/27/2019 Francisco Ruiz (Office Assistant) forward me a voice mail from Petty officer Singh. GAITAN Voice mail was requested information about sunken vessel at Kings Island Marina. Called number and left a voice mail. 12/30/2019 Contacted Sherri Keys, Office Manager for King Island Marina. Office GAITAN representative stated that Sherri is out of the office today and will return tomorrow. She transferred me to speak to the owner of the marina, Charles Spurlock. Charles stated that he does not know when Safety Kleen will pick-up the waste generated from the sunken vessel clean up. He stated that Sherri would know. Sent email to Sherri, I requested disposal receipts for waste generated. Printed out email and added to Emergency response folder. 04/22/2020 Complaint review. Sent email to Charles Spurlock, owner and Sherri Keys, GAITAN manager. Requested a copy of the disposal receipt for waste generated from 37-foot sunken vessel. Logged on to RCRA Online and found disposal receipt for one container (200 pounds)with absorbent contaminated with oil picked up on 1-22-2020. (007275143SKS)Accepted disposal receipt for waste generated from clean up of vessel. 04/23/2020 Contacted Charles Spurlock, property owner of Kings Island Marina. Discussed GAITAN disposal receipts for clean up of 37-foot sunken vessel. He stated that Sherri Keys, Office manager, has the disposal receipt and will send it to me tomorrow. Discussed status of the sunken vessel. He stated that the boat became under ownership of the insurance company. Insurance company paid vessel assist to tow the boat to Ladd's marina to scrap the boat for parts. Asked if he received any paperwork, he stated that he would ask Sherri to provide paperwork and contact information regarding the boat. 04/24/2020 Contacted Sherri Keys, Office Manager. She stated that she would be sending GAITAN over the manifest. I also requested paperwork for the removal of the vessel; she stated she would provide that information as well. 04/24/2020 Received email from Sherri Keys. Manifest attached was blank. Sent response GAITAN email requested a clearer copy or manifest number. She sent response email stated that she will contact safety kleen, due to not being able to read the manifest number herself. 05/18/2020 Contacted Sherri Keys, office manager. Discussed manifest/disposal receipt for GAITAN gasoline clean up and vessel removal by vessel assist.
