C3� I wul=R 20 12 <br />Once the above information is verified and correct, the Person -in -Charge will print, sign and <br />date the manifest. The StrongPak technician will also sign the manifest, then provide a copy <br />to the Person -in -Charge for store records. Obtain a copy of the manifest and file it in the <br />Manifests tab of your Hazardous Waste Program — Pharmacy binder. <br />Once the pickup is received at the disposal facility, a copy of the original manifest, now <br />signed by the disposal facility representative, is sent to the store via US mail (usually 3045 <br />days after the pickup). Obtain a copy of the second manifest and staple it to the copy of the <br />manifest provided at pickup, and file both copies in the Manifests tab of your Hazardous <br />Waste Program — Pharmacy binder. <br />Pharmaceutical Verification Form <br />At the time of pickup, the StrongPak technician <br />provides a Pharmaceutical Verification Form <br />verifying that the pharmaceutical waste tote <br />contains no identifiable DEA Controlled <br />Substances, identifiable DEA listed chemicals, <br />unidentified waste and/or pharmaceuticals that <br />carry a credit value as a manufacturer return. <br />The Pharmacy Manager is responsible for <br />reviewing the form to verify the following <br />information: <br />• The company name, store number and date <br />are correct. <br />The StrongPak technician name and manifest <br />number match the information on the <br />manifest. <br />Note: Inform the StrongPak technician if any of <br />the above information is incorrect and request a <br />corrected form. <br />EPdlt <br />Pharmaceutical Vorificntian Form <br />kaa.xnr <br />1MKulmacmraglnaturu'a ISfeE o. File the form in the <br />Pharmaceutical Verification Form tab of your Hazardous Waste Program — Pharmacy <br />binder. <br />Training <br />Every employee who will handle potentially hazardous Pharmacy waste or clean up <br />hazardous Pharmacy waste spills MUST be thoroughly trained. This means hands-on <br />training, detailed instruction, and periodic follow-up to ensure retention of learning. Training <br />for hazardous Pharmacy waste handling procedures is mandatory all Pharmacy personnel <br />and the Store Manager. <br />
