Site Safety and Health Plan <br />Stockton Field Maintenance Shop #24 <br />8020 Airport Way, Stockton, CA <br />7.0 MEDICAL SURVEILLANCE <br /> <br />7.1 PURPOSE <br />The purpose of a medical surveillance program is to provide uniform medical care, ensure the <br />selection of employees physically able to safely perform the work assigned, monitor employee <br />health on a regular basis and provide medical care for occupational injury or illness. This <br />program applies to all Star employees and subcontractors. Specific requirements of the program <br />are described below. <br /> <br />7.2 MEDICAL EVALUATION <br />All personnel involved in on-site operations must participate in an ongoing medical surveillance <br />program meeting the requirements of 29 CFR 1910.134 and 29 CFR 1910.120 (8 CCR 5192 <br />and 8 CCR 5144). The first examination will be conducted before personnel begin working at <br />the sites. The medical surveillance protocols and examination results will be overseen by a <br />licensed physician certified in Occupational Medicine by the American Board of Preventive <br />Medicine or who, because of necessary training and experience, is Board-eligible. The baseline <br />medical examination for the project will include the following: <br />Complete medical and occupational work history <br />General physical examination <br />Complete blood count <br />Electrocardiogram <br />Urine analysis with microscopic examination <br />Pulmonary function test <br />Chest x-ray <br />Audiogram <br />Visual acuity measurement <br />Ability to wear respirator based on the OSHA mandated respirator medical <br />questionnaire as defined in 29 CFR 1910.134 and 8 CCR 5144 <br />Stress test for employees over 40 years of age <br />This requirement will be strictly enforced for Star personnel, as well as all subcontractor <br />personnel. However, personnel currently participating in a medical surveillance program (with <br />yearly medical examinations), which meets the requirements of 29 CFR 1910.120 and 8 CCR <br />5192 relative to specific site conditions, will not be required to have additional medical <br />surveillance. Based on the results of this examination and other pertinent information, a <br />medical certification as to the fitness for employment on this project, or any restrictions on that <br />employee's ability to utilize PPE, will be maintained at the sites. However, specific medical <br />examination results will be maintained in strict confidence and will not be subject to disclosure <br />without the explicit written approval of the employee. <br />Beyond the general medical surveillance requirements listed above, no other chemical-specific <br />requirements are applicable to Star employees based on historical data from previous projects <br />and reasonable expectations of potential exposures at these sites. These requirements will be <br />revised accordingly if data reveal that chemical-specific action levels are being exceeded. <br />24 Star Resources Corp
