Site Safety and Health Plan <br />Stockton Field Maintenance Shop #24 <br />8020 Airport Way, Stockton, CA <br /> <br />7.3 ANNUAL EXAMINATION <br />Each Star employee who may be potentially exposed to waste materials, or who must wear a <br />respirator for more than 30 days per year, will receive an annual examination consistent with the <br />aforementioned baseline examination. A chest x-ray and electrocardiogram will not necessarily <br />be required during an annual examination, but rather will be determined collectively by the <br />occupational physician, the project Certified Industrial Hygienist, and the project SSHO. <br /> <br />7.4 SPECIAL MEDICAL EXAMINATIONS OR CONSULTATIONS <br />Special medical examinations or consultations will be arranged for all employees exposed to <br />hazardous substances at concentrations above the PEL without adequate protection. This will <br />be done as soon as possible after the overexposure has been determined. Exposures shall be <br />determined utilizing field instruments and personal exposure monitoring. <br />Considering the levels of various contaminants of concern (discussed in Section 3.0 of the <br />SSHP), the associated levels of PPE for site monitoring activities and the extensive sampling for <br />personal exposures we will be conducting, worker protection will be ensured through <br />implementation of the PPE/RPE program. If the PEL is anticipated to be exceeded, as <br />determined by field instrumentation, regardless of the use of PPE/RPE, then Star will evacuate <br />the work area and notify the CA ARNG's contracting officer and PM. <br />Special medical examinations will also be arranged upon notification by the employee that <br />he/she has developed signs or symptoms indicating a possible overexposure to hazardous <br />substances, or if the examining physician determines that more frequent medical examinations <br />are necessary. Non-scheduled medical exams may also be directed at the discretion of the <br />SSHO or the occupational physician. <br /> <br />7.5 EMPLOYEE NOTIFICATION OF MEDICAL EXAMINATION RESULTS <br />The medical consultant at Health Resources (a national medical surveillance program) will <br />review the results of the medical evaluation. If the examination uncovers a serious health <br />problem, the employee is to be contacted and notified in writing by the nurse at Health <br />Resources. Health Resources also recommends that the employee see his/her family doctor. If <br />the condition is more serious Health Resources will notify Star's human resources department. <br />If the health condition is pertaining to the job, Health Resources will conduct follow-up <br />monitoring. <br /> <br />7.6 SITE MEDICAL SERVICES <br />The employee performing the sampling is to be certified in first aid and CPR by the American <br />Red Cross (or equivalent). <br /> <br />7.7 MEDICAL PROVIDERS <br />In the event of an on-site injury requiring emergency medical care or, the following medical <br />centers will be used (Figure 2): <br />25 Star Resources Corp
