The current site maps in CERS do not address the location of all the <br />chemicals ( Sodium Hypochlorite, Phosphoric Acid, Oxygen, and Mineral Oi)l. The maps list Sodium Hydroxide. The <br />facility is no longer is using/storing Sodium Hydroxide. The site maps list Aluminum Sulfate as ALUM. The maps do <br />not list the emergency response equipment, and emergency shut offs. The site maps are not numbered, and 2 out <br />of 4 do not contain North Orientation. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. Any <br />abbreviations and symbols on the site map need to be addressed within the site map. Submit statement to EHD <br />stating that the business plan will be submitted to EHD Annually by the due date, January 15. <br />This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br />OBSERVATION: The Emergency Action Plan (EAP)/Emergency Response and Training Plan was not submitted <br />annually as a part of the business plan for years 2019 and 2020. Dale Klever, David Carter,and Bill Tapia have not <br />been working for this facility for longer than 30 days and the emergency contact and notification list with the EAP <br />has not been updated. The EAP does not list the emergency response equipment at this facility. <br />REGULATION GUIDANCE: The business owner/operator shall annually review and certify that the business plan is <br />complete, accurate, up-to-date and in compliance with Section 11022 of Title 42 of the United States Code. An <br />annual electronic submittal satisfies these requirements. The annual business plan shall be submitted/certified by <br />January 15 of each calendar year and may be submitted beginning November 1 of the previous year. <br />A business plan shall include the following emergency response procedures for a release or threatened release of <br />hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, review and submit/certify the <br />current and complete information.
