d. Monitoring Reauirements <br /> The permit requires continuous monitoring and recording of injection pres- <br /> sure, flow rate and volume, and annular pressure. Additional requirements have <br /> also been specified in Part II, Section C of the draft permit. The permit requires <br /> analysis of the injected fluid with sufficient frequency as to yield representative <br /> data of its characteristics. The monitoring requirements in the Class V permit are <br /> more stringent than those in the original Class I permit at the same site. <br /> e. Reporting Requirements <br /> The draft permit requires the applicant to report the results of all monitoring <br /> on a quarterly basis to the EPA and the Central Valley Regional Water Quality Con- <br /> trol Board. <br /> The draft permit requires the applicant to report the daily and monthly <br /> average, maximum and minimum values for injection pressure, flow rate and <br /> volume, temperature, and the annular pressure and annular fluid volume for the in- <br /> jection well. The draft permit also requires the applicant to report the results of the <br /> injection fluid analysis. <br /> In addition to routine quarterly reporting, the draft permit requires report- <br /> ing of the results of any mechanical integrity testing or any other well workover, <br /> logging, or testing that may reveal the condition of the casing, tubing, or packer. <br /> These reports are due within ninety days after the activity. This allows time for <br /> contractors and service companies to analyze data and to prepare a written report. <br /> f. Financial Responsibility <br /> The applicant has demonstrated the availability of financial resources to <br /> properly plug and abandon the well by means of a financial statement. If issued a <br /> permit, the applicant will be required to send updated information to the Regional <br /> Administrator within 90 days after the close of each succeeding fiscal year. <br /> g. Improperly Sealed or Abandoned Nearby Wells <br /> Two oil and gas wells are known to penetrate the the proposed injection zone <br /> within the area of review (3 mile radious from the proposed well) . Both these <br /> wells are properly plugged according to California Division of Oil and Gas reports. <br /> Therefore, no corrective action is required. The Agency acknowledges that other <br /> abandoned wells may exist in the vicinity, however these do not appear to present a <br /> threat to underground sources of drinking water. <br /> h. Plugging and Abandonment <br /> The draft permit requires the permittee to notify the Director no later than <br /> 45 days before conversion or abandonment of the well. The permittee is required to <br /> plug and abandon the well in the manner prescribed in the plugging and abandon- <br /> ment plans. Within 60 days after plugging the well, or at the time of the next quar- <br /> terly report (whichever is shorter), the permittee shall submit a report to the Direc- <br /> tor. The report shall be certified by the person who performed the plugging opera- <br /> tion, and shall consist of either: <br /> 8 <br />
