(1) a statement that the well was plugged in accordance with the plan previ- <br /> ously submitted to the Director; or <br /> (2) if the actual plugging differed from the approved plan, a statement <br /> describing the actual plugging and reasons why the Director should approve such a <br /> deviation. Any deviation from a previously approved plan may be cause for the <br /> Director to require the operator to replug the well. <br /> The draft permit requires the permittee to plug and abandon the well two <br /> years after cessation of injection in accordance with the plan unless the permittee: <br /> (1) provides notice to EPA; and <br /> (2) describes actions or procedures, which are approved by the EPA, to <br /> ensure that the well will not endanger USDWs during the period of tem- <br /> porary abandonment. These actions and procedures shall include compliance <br /> with the technical requirements applicable to active injection wells unless <br /> waived in writing by EPA. <br /> 10. THE ADMINISTRATIVE RECORD <br /> As required by 40 CFR 124.9, the administrative record, including the application, <br /> the draft permit, and the data submitted by the applicant, is located and available <br /> for public inspection at Region 9 EPA Library, 1235 Mission Street, San Francisco, <br /> CA 94103. A copy of the draft permit and fact sheet are available for inspection at <br /> the Fresno Public Library, 2420 Mariposa Street Fresno, California and at the Han- <br /> ford Public Library, 401 North Douty Hanford, California. <br /> 11. REFERENCE DOCUMENTS <br /> All documents used in the preparation of the draft permit are available at EPA, <br /> Region 9. Information regarding these materials may be obtained from the person <br /> listed below. <br /> 12. EPA CONTACT : <br /> Steve Ihnen <br /> c/o Underground Injection Control Section, W-6-2 <br /> (415) 705-2099 <br /> (415) 705-2111 (leave message) <br /> 13. PROCEDURES FOR THE FORMULATION OF FINAL DETERMINATIONS <br /> EPA proposes to issue a UIC permit to this applicant, subject to the aforementioned <br /> limitations and special conditions. This determination is tentative and open to com- <br /> ment from the public from July 2, 1990 until August 7, 1990. <br /> 9 <br />
