The following is an itemized list of underground storage tank violations that have not been addressed for SINCLAIR/DINO MART as of July 19 , 2021. Open violations from May 26 , 2021 inspection Violation #122 - Failure to implement corrections and submit written response within 30 days of receiving the report. OBSERVATION An inspection was last done on May 26 , 2020 and an inspection report was issued identifying information to be submitted to bring this site into compliance . This information was required to be submitted by June 25 , 2020 . This information has not been received resulting in a non -compliant status for this facility. REGULATION GUIDANCE: An operator that receives an inspection report shall have 30 days to implement the corrections specified in the inspection report and comply with permit conditions . The corrective action shall include all of the recommendations made in the report. The operator shall submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken . CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is submitted and all corrections implemented within 30 days of receiving an inspection report. ❑ This violation was corrected ❑ This violation will be corrected by ( date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #203 - Failure to maintain monitoring records for 36 months and maintenance records for 60 months. OBSERVATION Maintenance and monitoring records for the last three years were not found on site . The following documents were not located : -May 26 , 2020 monitoring system certification -May 30 , 2019 monitoring system certification - May 24 , 2018 monitoring system certification REGULATION GUIDANCE: These records shall be maintained on site for at least three years . Monitoring records include : ( 1 ) date and time of all monitoring or sampling ; (2 ) monitoring equipment calibration and maintenance records ; (3 ) results of any visual observations ; (4 ) results of sample analysis performed a lab or in the field ; ( 5 ) logs of all readings of gauges or other monitoring equipment , ground water elevations , or other test results ; (6 ) results of inventory readings and reconciliations .
