The following is an itemized list of underground storage tank violations that have not <br /> been addressed for SINCLAIR/DINO MART as of July 19 , 2021 . <br /> Open violations from May 26 , 2021 inspection <br /> Violation #205 - Failure to have current UST Monitoring Plan readily accessible at the facility . <br /> OBSERVATION An approved copy of the monitoring plans were not available for inspection . <br /> REGULATION GUIDANCE: A copy of these plans shall be accessible on site at all times . <br /> CORRECTIVE ACTION : Immediately locate and retain a copy on site . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #207 - Failure to maintain designated operator inspection reports and all attachments on site for <br /> 36 months. <br /> OBSERVATION The June 2019 designated operator inspection report was not found on site . <br /> REGULATION GUIDANCE: Designated operator inspection reports prior to October 1 , 2018 shall be retained on <br /> site for twelve months . Designated operator inspection reports on or after October 1 , 2018 shall be retained on site <br /> for thirty-six months . <br /> CORRECTIVE ACTION : Locate and ensure that copies of the previous thirty-six months of designated operator <br /> inspection reports are maintained on site . Submit copies of the missing reports to the EHD . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #208 - Failure to comply with the designated operator inspection requirements . <br /> OBSERVATION The latest designated operator report on site was dated April 1 , 2020 . The facility did not have <br /> designated operator reports for the following months : May 2020 , June 2020 , July 2020 , August, 2020 , September <br /> 2020 , October 2020 , November 2020 , December 2020 , January 2021 , February 2021 , March 2021 , April 2021 , and <br /> May 2021 . <br /> REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br /> the previous inspection . <br /> CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br /> frames. <br /> Page 4 of 8 <br />
