The emergency coordinator and <br />modified contingency plan information is outdated. Tony Mansfield is <br />listed as the emergency coordinator; according to Mr. Shayne Norman, <br />Mr. Mansfield has "been gone a little over a year." There must be at <br />least one emergency coordinator on site or on call to coordinate <br />emergency response measures, and the following information must be <br />posted by a phone: the name and phone number of the emergency <br />coordinator; location of fire extinguishers, spill control equipment, <br />and if present, fire alarm; and the phone number of the fire <br />department, unless the facility has a direct alarm. OBSERVATION: Hazardous waste <br />containers were observed on-site with incomplete labeling. The <br />containers were: -Three black metal 55 gallon drums were missing the <br />words "Hazardous Waste", generator name & address, accumulation start <br />date and physical state. The hazardous waste label was on the wall <br />behind the three drums of used oil. -One approximately 20 gallon black <br />metal drum of oily solids was observed with a faded, unreadable <br />hazardous waste label. REGULATION GUIDANCE: All hazardous waste <br />containers shall be marked with the following information: the words G <br />Hazardous WasteG , name and address of generator, hazardous <br />properties, physical state, composition (contents), accumulation start <br />date CORRECTIVE ACTION: Immediately label the containers as required <br />for hazardous waste.
