Site Health and Safety Plan <br />Countryside Market, Stockton, California March 21, 2014 <br />of the Health and Safety Director. Some of the Health and Safety Director's specific responsibilities <br />include: <br />Advising the Project Manager and Site Health and Safety Coordinator on matters relating to <br />health and safety on this project. <br />Coordinating field audits to monitor the effectiveness of this HASP and to assure <br />compliance with it. <br />Supporting the Project Manager as needed to evaluate site conditions and new information <br />which might require modifications to the HASP. <br /> <br />2.2 Minimum Training, Respirator Fit-Testing, and Medical Surveillance Requirements <br />for Apex-SGI Personnel <br />40 hr. Hazardous Waste Operations Training (HAZWOPER) 29CFR1910.120. <br />8 hr. Annual HAZWOPER Refresher Training (current). <br />8 hr. Supervisor HAZWOPER Training for Site Health and Safety Coordinator. <br />Annual Respirator Fit Testing. <br />Annual Medical Clearance and Respirator Clearance by a physician. <br /> <br />2.3 Site Visitor <br />Occasionally, visitors may arrive at the Site during field activities. In general, most visitors can be <br />accommodated by providing a viewing area in a safe location away from the active work zones and <br />presenting a briefing conducted by the Site Health and Safety Coordinator. All visitors will sign into <br />the Site. All visitors will read, understand, and sign the Health and Safety Acknowledgement and <br />Agreement Form (Appendix B), acknowledging they have read and understand the HASP. <br />In some instances, visitors may require access to restricted zones of the Site. If a visitor desires <br />access to the Site, the Site Health and Safety Coordinator will make arrangements for entry. The <br />required level of PPE within the exclusion area will be strictly adhered to. Visitors must be escorted <br />at all times. Visitors who cannot show proof of the required documentation of employee training <br />and participation in a medical monitoring program as mandated by California Occupational Safety <br />and Health Administration (CAL-OSHA) will not be allowed access to the exclusion areas. If <br />respiratory protection is required, the visitor must furnish their own, and the respirator type must <br />match the specifications detailed in this HASP. <br />If a workable, safe arrangement cannot be agreed upon, or if the activities of the visitor jeopardize <br />Site activities, the Site Health and Safety Coordinator should immediately contact the Project <br />Manager, who may decide to stop work activities immediately. <br />A <br />HASP 21MAR2014 <br /> 2-3 <br /> APEX
