Site Health and Safety Plan <br />Countryside Market, Stockton, California March 21, 2014 <br />3.0 GENERAL HEALTH AND SAFETY PROCEDURES <br />Utility Clearance will be completed prior to beginning any subsurface work. <br />The Site Health and Safety Coordinator is responsible to ensure that the health and safety <br />plan procedures are followed. Any subcontractor or other person subject to the plan will be <br />dismissed for failure to comply. <br />Daily tailgate safety meetings will be held by the Site Health and Safety Coordinator. <br />All personnel who will be working in the contamination control zone will undergo an <br />industrial hygiene baseline medical examination before commencing work. This <br />requirement does not apply to individuals who have taken the examination during the last <br />12 months. <br />The same personnel must be fitted and fully instructed on the use of respirators prior to <br />starting work. Beards or other facial hair that interfere with respirator fit are prohibited for <br />those individuals who may be required to use respiratory protection. <br />Hearing protection in the form of disposable earplugs will be worn around heavy <br />equipment, machinery, or when two individuals five feet or less apart need to shout to be <br />heard. <br />Potable water must always be available at the Site. <br />Hands must be thoroughly washed upon leaving the work area and before eating, <br />drinking, or any other activity. <br />If toilet facilities are not located within a 5-minute walk from the decontamination facilities, <br />either provide a chemical toilet and hand washing facilities or have a vehicle available (not <br />the emergency vehicle) for transport to nearby facilities. <br />Establish Exclusion Zone, and set up Contamination Reduction Zone and Support Zone <br />when upgrading to Level C. <br />If necessary, provide dust control by spraying soils with water or a surfactant/water solution. <br />Perform regular air monitoring in working zone. <br />Use ground fault circuit interrupters for plug-in electrical devices and extension cords. <br />Be aware of tripping hazards with extension cords, tools, hoses, augers, etc. <br />Eating, drinking, chewing gum or tobacco, smoking, or any practice that increases the <br />probability of hand-to-mouth transfer of materials is prohibited in the work areas. <br />Contact with contaminated or suspected contaminated surfaces should be avoided. <br />Whenever possible, don't walk through puddles, mud and other discolored surfaces; don't <br />A <br />HASP 21MAR2014 <br /> 3-1 <br /> APEX
