Remediation Bid <br />David Parker <br />Holt, California <br />May 7,2012 <br />Proposal No. 1085 <br />Page 2 <br />Based on my survey of the site, I assume that 1,000 square feet of soil will need to be removed to <br />a depth of 1 foot below grade (approximately 40 CY). After this soil has been excavated, <br />confirmation soil samples will be collected at the base of the excavation to ensure the soil has <br />been remediated to the County approved cleanup levels. The excavated soil will be placed in <br />roll-off boxes and transported to the Hay Road Landfill near Dixon, CA. The asbestos material <br />will be triple bagged and appropriately disposed of as friable asbestos waste at the Hay Road <br />Landfill. <br />The following scope of work describes the necessary tasks to remediate the property: <br />Scope of Work <br />Project Preparation <br />Prepare a Remediation Work Plan and submit to the SJCEH <br />Prepare a site specific health and safety plan <br />Notify USA Alert at least 48 hours prior to commencing fieldwork <br />Notify SJCEH at least 48 hours prior to commencing fieldwork <br />Obtain temporary EPA ID number for disposal of hazardous wastes <br />Obtain additional lab analyses to profile the waste <br />Submit profile fees to Hay Road landfill <br />Coordinate with the landfill regarding disposal date(s). <br />Site Work <br />Mobilize equipment to the site <br />Remove and properly dispose of up to 40 CY of oily soil under a non-RCRA hazardous <br />waste manifest. <br />Remove and properly dispose of up to 1 CY of friable asbestos waste (including pipes) <br />Collect up to eight (8) soil samples from the base of the excavation and analyze for <br />TRPH and TPH-d. All analyses will be conducted on a 24-hour turnaround basis. <br />Reporting <br />Prepare a Summary Report for submittal to the SJCEH <br />Total Price: $21,175.34 <br />An itemized cost estimate is attached. Also attached are our exclusions, clarification, terms, & <br />conditions specific to this proposal. Please note that this proposal is based on personal <br />observations and lab results from a survey conducted on April 20, 2012, as well as verbal <br />information provided by you during the same site survey. In the event the scope of work <br />required to perform the referenced work differs from that which CES has bid, CES shall be <br />entitled the opportunity for price adjustment(s) to account for the differences.
