CHOW ENGINEERING, INC. <br />Rev. 6, 8-8-14 <br /> <br />6.0 Site Controls, Procedures and Practices <br /> <br />6.1 Controlled Access Zones <br />To decrease safety risks CE restricts access to the work areas on all of its sites. A <br />controlled access zone (CAZ) shall be clearly defined by the crew superintendent as an <br />area where a recognized hazard exists. The demarcation of the CAZ shall be <br />communicated by the crew superintendent in a recognized manner, either through <br />signs, fencing, wires, tapes, ropes or chains. <br />Chow Engineering shall take the following steps to ensure that the CAZ is clearly <br />marked and controlled: <br />All access to the CAZ must be restricted; only authorized entrants shall be <br />allowed entry; <br />All workers who are permitted in the CAZ shall be listed in the appropriate <br />sections of the Plan (or be visibly identifiable by the competent person) prior <br />to entry; <br />The site supervisor shall ensure that all protective elements of the CAZ be <br />implemented prior to the beginning of work. <br /> <br />6.2 General Safe Work Practices <br />CE personnel are responsible for and conduct safe work practices at all times. Work <br />will be performed under the guidelines of CE SOP #H&S 002— "Safe Work Practices". <br />This includes addressing the work scope to be done at the start of each day or each <br />shift with all personnel at a "Tailgate Safety Meeting". CE's supervisors are required to <br />monitor work conditions on site at all times and to make required adjustments to the <br />safety protocols as required. <br /> <br />6.3 "Tailgate" Safety Meetings <br />CE personnel will conduct and document daily tailgate safety meetings, at a minimum <br />occurring prior to commencement of operations at each shift change, to be attended by <br />all personnel working in or visiting the operations area, to include all subcontractors. <br />Tailgate safety meetings will address the specific tasks and associated hazards related <br />to the work to be performed that shift. Tailgate meetings will be documented as <br />described in CE SOP #H&S 003. On the form described, and all participants will sign <br />the form sheet to document attendance and comprehension of the topics, plans and <br />procedures discussed. <br />6
