S0 JOA U 1 Environmental Health Department <br /> U ; i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ALL STAR HEAVY HAUL&TOW 1 17830 S IDEAL PKWY, MANTECA October 03, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 113 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> OBSERVATION: Copies of consolidated manifest receipts for hazardous wastes including used oil, and household <br /> hazardous waste were not found on site during the inspection <br /> REGULATION GUIDANCE: The generator shall retain each consolidated manifest receipt for at least three years. <br /> This period of retention is extended automatically during the course of any unresolved enforcement action <br /> regarding the regulated activity or as requested by the department or a certified unified program agency. <br /> CORRECTIVE ACTION: Immediately locate a copy of all consolidated manifest receipts from the past three years <br /> and maintain them on site. Provide a copy of the consolidated manifest receipts and a corrective action statement <br /> to the San Joaquin County Environmental Health Department. <br /> This is a Class II violation. <br /> 115 CCR 66266.81(a)(4)(B)Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> OBSERVATION: Fourteen lead acid batteries were observed in one of the shipping containers and one lead acid <br /> battery was observed next to the waste dumpster. Bills of lading for the recycling of lead acid batteries from were <br /> not found on site. <br /> REGULATION GUIDANCE: <br /> (A)The generator shall use either the manifest or a bill of lading which fulfills the requirements of Title 13 CCR <br /> section 1161,to record the shipment of spent lead-acid storage batteries to a person who stores the batteries or <br /> who uses, reuses, recycles or reclaims the batteries or their components. <br /> (B)The generator shall retain, at the generator's place of business for at least three years, a legible copy of each <br /> manifest or bill of lading which identifies spent lead-acid storage batteries shipped to a person who stores the <br /> batteries or who uses, reuses, recycles or reclaims the batteries or their components. <br /> CORRECTIVE ACTION: Provide copies of battery core bill of lading records and a corrective action statement to <br /> the San Joaquin County Environmental Health Department within 30 days. <br /> This is a Class II violation. <br /> 119 HSC 25404(e)(4) Failed to report program data in the California Environmental Reporting System (CERS). <br /> OBSERVATION: This business's hazardous waste activities have not been reported on the California Electronic <br /> Reporting System (CERS)website. <br /> REGULATION GUIDANCE: Each regulated business shall report program data electronically on the CERS <br /> website. <br /> CORRECTIVE ACTION: Immediately report your business activities including the hazardous waste program on the <br /> CERS website.
