POLICY/PROCEDURE: SM 2.0.01 <br /> TOPIC: EMERGENCY PREPAREDNESS PLAN <br /> GENERAL/CHAIN OF COMMAND: <br /> 1.0 Any employee who is not at work or scheduled to work at the time the disaster occurs will be asked to call or come into <br /> the facility if it is safe to do so once designated calls to other staff members on the telephone tree are completed. <br /> 2:0 Between the hours of 7:00p.m.and 8:00 a.m.daily or other times(Sundays,Holidays)when no one is at the facility: <br /> 2.01 The Administrator or designee will contact the department directors to activate the telephone tree. <br /> 2.2 Key employees will be requested to come to the facility to coordinate field activities.This will include <br /> the Warehouse supervisor,and a minimum of two drivers. <br /> 2.03 Available drivers are to see patients and to deliver needed pharmaceuticals,oxygen,and <br /> supplies if it is safe to do so. <br /> IF PHONE SERVICES ARE NOT OPERATING: <br /> 1.0 The on-call pharmacist will come to the facility office. <br /> 2.0 The on-call clinicians and/or Patient Care Coordinators will activate the telephone tree,if operational,and then will <br /> review their caseloads.Clinicians will visit patients as soon as possible. <br /> 3.0 In the event of the Administrator's absence or unavailability,internal staff will be directed by: <br /> 3.01 Department Directors or designees <br /> 3.02 Supervisors <br /> 4.0 In the event the Administrator is unavailable after regular business hours,procedure will be directed by: <br /> 4.01 Department Directors or designees <br /> 4.02 Supervisors <br /> DISRUPTED PHONE SERVICE GUIDELINES: <br /> PURPOSE: <br /> To provide continuity of patient care in the event of a disruption in phone service. <br /> POLICY: <br /> Guidelines for disrupted phone service will be reviewed annually. <br /> PROCEDURE: <br /> 1.0 Check the phone system. <br /> 1.01 If not working,gather as many car phones as possible for use by the staff. <br /> 1.02 Activate use of dedicated phone/fax lines. <br /> 1.02.02 Pharmacy- (818)956-6317 <br /> 1.02.02 Billing- (818)956-6695 <br /> 1.02.03 Accounting (818)5461579 <br /> 1.02.04 Respiratory (818)844-5182 <br /> 2.0 Western Drug Medical Supply's phone company's is AT&T.. Inform them of the problem immediately.Don't <br /> assume that another individual has done so until so informed. <br /> 3.0 Call the exchange;inform them of the problem.Have them take all calls until further notice.Any urgent calls should be <br /> forwarded to Western Drug Medical Supply via pagers,or dedicated phone lines. <br /> 4.0 When phones are working again,inform exchange,and retrieve messages. <br />
