Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: C00043251 Site Location: 833 E EIGHTH ST <br />Activity Summary <br />Activity Date Recorded by <br />04/12/2017 I discussed with Cesar( Covering ER/C) the follow up with this incident (cleanup SAEED <br />scheduled this morning) and he told me that he is going to UST inspection. I <br />discussed it with Elena , and Elena told me to follow up with the cleanup. <br />04/12/2017 At 09:17 am I called scott to follow up with the cleanup and he told me that he just SAEED <br />arrived on site , and he will call me in a bit to update me on the cleanup status. At <br />09:49 am Scott called and told me that the excavation is still going and there are <br />more areas to be excavated. Scott told me that he will notify me at the end so I <br />can come and take a look. I told Scott I will be there within an hour to see the <br />excavations before it rains. <br />At 11:00am I left the office to go to the site. I arrived at 11:08am and met with <br />Scott. Scott and I inspected the spills. Some of the excavated areas still have oily <br />water and black oily stain. Scott told me that the deepest they could go without <br />jeopardizing the integrity of the tracks. Scott told me that the dark stain is possibly <br />old residuals have accumulated over years, and the PID shows zero reading. <br />these excavated areas about 2 feet deep. Scott mentioned that he will take <br />samples to show that the dark stain is old residuals. Some of the contaminated <br />areas has not been cleaned up yet and I point them to Scott. I asked Scott about <br />his plan and told me to back fill the excavated spots and placing along sheet of <br />absorbent pads on the top to prevent the same thing happening in the future, and <br />these will be added to the maintenance routine inspection. I told Scott to contact <br />Paul and confirmed we all in the same page. Scott called Paul and told me that <br />Paul agreed with him. <br />At 11:40am I called Muni , and told him about the situation and explained that I still <br />see black stain inside the excavated areas, and told him that Scott told me that <br />how far they can dig without jeopardizing the integrity of the tracks. Muni told me if <br />thats their plan, then we will documented. I asked Muni if he can come to evaluate <br />the situation, and he told me there is no need. Scott told me that they will continue <br />the cleanup and he will notify me when they about to finish. I took photos and and <br />at 11:58am I left the site. <br />1:20 I added the the photos to the incident file. I created photo pages, and labeled <br />the photos. I showed the photos to Muni. <br />At 1:45 pm I completed the narratives for this part. <br />At 2:27pm Scott with Arcadis called and told me that they are finishing up with <br />excavation , and I can go and take a look. At 2:39 pm I arrived on site. Cesar( <br />Trainee) arrived at 2:42pm. We inspected the cleanup and took photos. There <br />were two spots toward the Diesel Shop that need to be more excavated, and <br />pointed them to scott. Scott asked the crew to excavate these two spots until they <br />reached about 2 feet deep ( the rocks and the soil at these level still looks stained <br />and black like the other excavations, but these the limit scott told me they can dig). <br />Six samples were taking to be analyzed from different areas. I took photos, and <br />we left the site at 3:38pm. <br />04/13/2017 I transfered the photos I took during my second site visit yesterday (4.12.17). I SAEED <br />created photo pages and labeled them. I printed the photos to be filed. <br />5105 rpt <br />Page 3 of 5
