Complaint Investigation Summary Report #. 5105 COMPLAINT ID: C00043251 Site Location: 833 E EIGHTH ST Activity Summary Activity Date Recorded by 04/12/2017 I discussed with Cesar( Covering ER/C) the follow up with this incident (cleanup SAEED scheduled this morning) and he told me that he is going to UST inspection. I discussed it with Elena , and Elena told me to follow up with the cleanup. 04/12/2017 At 09:17 am I called scott to follow up with the cleanup and he told me that he just SAEED arrived on site , and he will call me in a bit to update me on the cleanup status. At 09:49 am Scott called and told me that the excavation is still going and there are more areas to be excavated. Scott told me that he will notify me at the end so I can come and take a look. I told Scott I will be there within an hour to see the excavations before it rains. At 11:00am I left the office to go to the site. I arrived at 11:08am and met with Scott. Scott and I inspected the spills. Some of the excavated areas still have oily water and black oily stain. Scott told me that the deepest they could go without jeopardizing the integrity of the tracks. Scott told me that the dark stain is possibly old residuals have accumulated over years, and the PID shows zero reading. these excavated areas about 2 feet deep. Scott mentioned that he will take samples to show that the dark stain is old residuals. Some of the contaminated areas has not been cleaned up yet and I point them to Scott. I asked Scott about his plan and told me to back fill the excavated spots and placing along sheet of absorbent pads on the top to prevent the same thing happening in the future, and these will be added to the maintenance routine inspection. I told Scott to contact Paul and confirmed we all in the same page. Scott called Paul and told me that Paul agreed with him. At 11:40am I called Muni , and told him about the situation and explained that I still see black stain inside the excavated areas, and told him that Scott told me that how far they can dig without jeopardizing the integrity of the tracks. Muni told me if thats their plan, then we will documented. I asked Muni if he can come to evaluate the situation, and he told me there is no need. Scott told me that they will continue the cleanup and he will notify me when they about to finish. I took photos and and at 11:58am I left the site. 1:20 I added the the photos to the incident file. I created photo pages, and labeled the photos. I showed the photos to Muni. At 1:45 pm I completed the narratives for this part. At 2:27pm Scott with Arcadis called and told me that they are finishing up with excavation , and I can go and take a look. At 2:39 pm I arrived on site. Cesar( Trainee) arrived at 2:42pm. We inspected the cleanup and took photos. There were two spots toward the Diesel Shop that need to be more excavated, and pointed them to scott. Scott asked the crew to excavate these two spots until they reached about 2 feet deep ( the rocks and the soil at these level still looks stained and black like the other excavations, but these the limit scott told me they can dig). Six samples were taking to be analyzed from different areas. I took photos, and we left the site at 3:38pm. 04/13/2017 I transfered the photos I took during my second site visit yesterday (4.12.17).
