14A/15B UDC sump was due to a break in the flex hose connection, which was <br /> incorrect, since photos taken during the inspection shows diesel leaking from the <br /> dispenser itself. Walton Engineering, Inc. was hired to investigate the source of fuel in <br /> the piping sumps. Maintenance records not found during the routine UST inspection <br /> were not submitted. <br /> On November 23, 2009, Ms. Henry called Mr. Gill and told him that not all of the <br /> violations were addressed. Mr. Gill stated he did not have the paperwork in front of him <br /> so Ms. Henry did not review each violation in detail. Ms. Henry reminded Mr. Gill that <br /> permit fees were still not paid and they do not have a permit to operate. Ms. Henry also <br /> pointed out that the monitoring plan submitted had errors, and Mr. Gill stated that was all <br /> the information he had but would check with Walton Engineering. Ms. Henry asked <br /> when she could expect the remaining issues to be corrected, and Mr. Gill replied that <br /> since it was a short week, maybe the following week. Ms. Henry reminded Mr. Gill that <br /> the monitoring system certification for the automobile side of the facility was not done yet <br /> and would be a billable inspection. <br /> On December 2, 2009, Ms. Henry returned a call from Mr. Gill and asked him about the <br /> overdue annual permit fees. Mr. Gill stated they have been paid, and he had a <br /> cancelled check from the EHD. Ms. Henry stated that it may have been for the food <br /> permit and gave Mr. Gill the EHD Accounting Office telephone number. Mr. Gill stated <br /> that Walton Engineering would be performing the monitoring system certification for the <br /> automobile side of the facility later this week, and Ms. Henry told him that nothing had <br /> been scheduled with the EHD. Ms. Henry asked when the remaining issues would be <br /> corrected and Mr. Gill stated that Walton Engineering was working on it. <br /> On December 28, 2009, Ms. Henry witnessed the annual monitoring system certification, <br /> leak detector testing, and spill container testing of the automobile side of the facility. <br /> Liquid was found in all the UDC sumps, the 8,000-gallon diesel UST fill and piping <br /> sumps, and the vent transition sump. The 87-octane piping sump interstitial sensor <br /> failed when tested and was replaced on site. Ms. Henry reviewed the facility's <br /> paperwork and found a December 21, 2009, work order from Walton Engineering that <br /> stated, "Dispenser 14 has a severe leak. Remove approx five gallons of fuel from the <br /> UDC." Ms. Henry asked to see the sumps at the truck stop side of the facility and found <br /> a visible leak at dispenser 14A/15B, and liquid in the piping sumps for both of the <br /> 20,000-gallon diesel USTs. The diesel leak detector at the automobile side could not be <br /> tested due to low fuel levels, and the 91-octane piping sump interstitial sensor did not <br /> come out of alarm after testing. The automatic tank gauges on the automobile side were <br /> tested, but the test report submitted incorrectly states that the tank gauging equipment <br /> was used only for inventory purposes and not leak detection monitoring (Attachment 31). <br /> The automatic tank gauges on the truck stop side were not tested. A current monitoring <br /> plan still had not been submitted. An inspection report was mailed to Fast Lane Central <br /> Valley, LLC, in Healdsburg, CA (Attachment 32). <br /> On December 28, 2009, Ms. Henry also performed a hazardous waste reinspection <br /> since all the violations found during the October 19, 2009, routine hazardous waste <br /> inspection were not corrected (Attachment 33). A 30-gallon drum of "diesel waste" was <br /> found labeled, but missing the hazardous properties and physical state. Records of <br /> universal waste disposal records were still not found on site. <br /> 7 <br />
