During the inspection in June 2012, it was observed that Designated operator failed to document all <br />alarms for the previous months and check that they were responded to appropriately on the monthly <br />inspection reports. The truck side missing alarms not documented on the November and October 2011 <br />monthly reports include: <br />-1-10 Diesel 2 Annular N. annular space. fuel alarm & sensor out on 10/07/11 at 12:56PM, 12:50Pm. <br />10/06/11 at 6:23PM <br />-1-14 Dispenser 13 brine. annular space. fuel alarm and sensor out on 11/03/11 at 9:11 PM <br />These alarms also lack a written response action although work orders were found unattached: <br />-WO1188363 from Service Station System dated 10/07/11. <br />-WO1187836 from Service Station System dated 11 /04/11. <br />Designated operator documented some alarms but no response action was written nor was a work/ <br />repair owner attached to the reports for the following alarms: <br />-S2 No vacuum alarms on 06/05/11, 07/07/11, 07/08/11, 09/01/11. <br />-L10 Diesel 2 Annular N. annular space. high liquid alarms on 07/01/11 and 07/02/11. <br />-S1 an S2 communication alarms on 08/05/11. <br />-S1 high liquid alarm and no vacuum on 12/02/11. <br />During the monthly inspection, the designated operator shall review the alarm history for the previous <br />month, check that each alarm was documented and responded to appropriately, and attach a copy of <br />the alarm history with documentation taken in response to any alarms on the monthly report. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. Submit a written document describing actions taken in response to the above <br />alarms. <br />201. During the inspection in June 2012, it was observed that the alarm sensor log was not found on <br />site. Any and all alarms must be recorded by employees if they were not documented by the designated <br />operator. No documentation on alarm logs or monthly reports can be found for the above alarms <br />referenced in violation #204. These records shall be kept on site for at least three years. Immediately <br />locate maintenance records/ work orders and maintain the alarm sensor logs on site. Work orders <br />should be attached to the designated operator's monthly reports. Submit copies of these maintenance <br />records to EHD associated with the above alarms in violation #204 along with explanation how <br />employees are keeping record of the alarms. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received By: <br />Title: <br />Thuy Tran (209) 468-8257 <br />EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
