Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COAA53LY6 Site Location: 14900 W HVVY 12 <br />Activity Summary <br />Activity Date Recorded by <br />07/30/2020 6:01 PM Called Tow Boat US 925-382-4422. Someone answered my phone call MURO <br />but was not speaking. I called Tommy Mendeleski and he told me Tow Boat US <br />just arrived at the marina. I asked if he had an idea of how long it will take to <br />remove the vessel from the water. He asked Tow Boat US personnel, they <br />approximated this will take 1.5 hours. I asked if he can give me a call to let me <br />know once they lifted the vessel out whether there is a release or not. He said he <br />would. <br />6:06 PM Called Fish & Wildlife Fairfield dispatch. I asked if they received the Cal <br />OES Control #: 20-4054 Potential Release Spill Report. They did. I left my <br />information if they needed to contact me. <br />6:11 PM Called Coast Guard dispatch person, Katelyn Clifford, 707-374-2871: I <br />asked if they received the Cal OES Control #: 20-4054 Potential Release Spill <br />Report. She took my contact information and said she will look into it and call back <br />if any questions. <br />6:51 PM Received Page form SJGH Operator advised of incoming call so she <br />would call me back. <br />7:09 PM Didn't hear from SJGH Operator, I called back. Operator reported that <br />Tommy Mendleski called to report a release at Tower Park Marina. I told her I <br />already had the contact information and that I will call Tommy Mendleski. <br />7:11 PM Called Tommy Mendleski (Tower Park Marina) and he reported that <br />when Tow Boat USA lifted the boat out of the water, he saw a 4 X 4 ft square of <br />sheen. He said if I was going to head out there, the sheen will probably be gone by <br />then since it was so minuscule. <br />7:15 PM Called available lead senior supervisor, Haza Saeed (EHD). He advised I <br />head out there. I put on my PPE, gathered my backpack and headed out to Tower <br />Park Marina. <br />8:15 PM Arrived at Tower Park Marina. Met with Tommy Mendleski. He escorted <br />me to the site of sunken vessel. I did not observed any sheen in the water, or <br />away in the near distance, or on the boom. Tommy Mendleski was in a hurry as it <br />was getting dark and went to show me to the Responsible Parties (RP), the vessel <br />owners, Jehari and Sean Bally. They showed me the vessel, which was parked on <br />the asphalt. I noticed there was an area of some sort of released liquid under the <br />vessel's motor. I did not observe an active leak, just a wet area. I wasn't certain if it <br />could be fuel. I did not detect a smell of fuel in the air. I took photos and consulted <br />Haza Saeed. Haza advised that the RPs place a container to collect the liquid. <br />Haza Saeed asked that I send out a notice to abate for waste determination for the <br />liquid and for any waste generated. <br />Left site <br />Generated Complaint and Notice to abate. Emailed notice to abate to responsible <br />party and cc'd Tommy and Elena Manzo. <br />07/30/2020 Received new complaint information from Elianna Florido, REHS and a directive MURO <br />from Elena Manzo, Lead Senior Supervisor, REHS. <br />07/31/2020 Received update on spill from Claudia Muro, EHS.
