Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COAA53LY6 Site Location: 14900 W HVVY 12 <br />Activity Summary <br />Activity Date Recorded by <br />07/31/2020 Discussed complaint with Elianna Florido, Jason Gaitan, and Lead Senior Stacy MURO <br />Rivera. <br />Retrieved voice message from Fish and Wildlife Officer Landeen that was left this <br />morning at 7:31 am. Officer Landeen reported that the responsible party called <br />him back last night (7-30-20) at 9:30 PM to let him know they had removed the <br />boat from the water. Based on the information they gave him, he believed it was <br />not very impactful. He did not go out to the site. <br />Left EHD 8:59 AM Arrived at Tower Park Marina. Met with Tommy (Tower Park <br />Marina Manager). We went to look at the boom which was still in place. I did not <br />observe any oil or fuel staining on the surface of the boom. There was no sheen <br />on the water's surface in the slip or surrounding the slip. We walked to the vessel. <br />It was in the same parking space as last night. The container that was placed <br />under the vessel last night was dry. The asphalt under the containerwas dry. <br />There was no staining on either the container or asphalt. <br />Tommy Mendleski said he was still very new to his position at the marina. When <br />he first reported the "potential release" he was trying to be safe. He believes the <br />sheen he observed as they were re-surfacing the vessel, may have just been <br />residual from the boat, but turned out not to be an actual release. <br />Updated responsible party and let them know the I didn't find a release. Sean Bally <br />(RP) asked if it they can take the boat. I consulted Lead Senior Stacy Rivera, she <br />advised that I tell them we can make recommendations when something is not <br />safe to move, but we cannot prevent them from moving their vessel. I called Sean <br />Bally and her husband Jehari Bally (RP) answered. I told him what Stacy Rivera <br />advised. I let them know that I will be contacting them on Monday, once I consult <br />our Emergency Response Lead Senior Supervisor. <br />10:01 AM Left site. <br />After consulting Lead Senior Elena Manzo, I emailed Sean and Jehari Bally (RP) <br />at jeharibally@comcast.net to ask for the disposition of the fuel in the vessel's fuel <br />tank. Jehai Bally called back and let me know that the boat will be taken to a shop <br />for servicing, and will get back to us with fuel disposition. He believes that if the <br />fuel was mixed with water, that most likely the shop will be managing it. I let him <br />know I will need documentation whether it will be reused or disposed of. <br />07/31/2020 <br />08/03/2020 <br />MURO <br />MURO <br />MURO <br />MURO <br />FLORIDO <br />08/04/2020 Completed Narrative. <br />08/05/2020 Sent email to Lead Senior Elena Manzo (EHD) with details of my response and <br />question regarding billing for this complaint. <br />08/12/2020 Received a voice message from Isabella (916-724-9765) (she did not provide her <br />last name, nor agency) requesting an update on this incident; Isabella referenced <br />the Cal OES Report #20-4054. Looked up complaint. Discussed complaint <br />updates with Claudia Muro, EHS. Claudia to call and provide Isabella with an <br />update on the complaint. <br />5105.rpt Page 3 of 6
