Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: COAA53LY6 Site Location: 14900 W HVVY 12 <br />Activity Summary <br />Activity Date Recorded by <br />08/12/2020 Elianna Florido (EHD) received a call on her desk phone from Isabella MURO <br />(916-724-9765) requesting status information for Cal OES #20-4054 with incident <br />date 7-30-2020. Briefed Elianna on current complaint status. Called Isabella on <br />the number she provided. Left message with my contact info. <br />Isabella (Region 5 Water Quality Control Board) called back for update on the Cal <br />OES # 20-4054 spill report. I provided complaint details and my observations. I let <br />her know we were waiting on the disposition of the fuel that was on board the <br />vessel. <br />MURO 08/13/2020 Received two calls. <br />Sean Bally (RP) called to ask what further they need in regards to this incident. <br />Discussed billing. <br />Tommy (Tower Park Marina manager) called to ask if I can send something in <br />writing to document that there is nothing further. He stated the director of the <br />marina is concerned as the last incident costed them over $80,000, so they want <br />to ensure that in the future this incident doesn't bring on new claims. I told him my <br />Lead Senior Supervisor, Elena Manzo (EHD) is currently away and will return next <br />week. I will discuss with her. <br />08/17/2020 Received a call from Sean Bally on August 13 requesting that I contact the shop <br />for the fuel disposition that was on board the vessel. She provided shop <br />information: <br />Gone Fishin' Marine, 1880 N Lincoln St, Dixon, CA 95620. Shop Manager is Josh <br />Whitfield, phone number is: 707-678-1600. I called Josh Whitfield, the greeting <br />provided an email address, then the call went to voice mail. I emailed Josh <br />Whitfield at: josh@gfmarine.com, for status update on the vessel's fuel. <br />- Updated narrative. <br />08/18/2020 Lead Senior Elena Manzo (EHD) responded to my question on whether this <br />complaint was chargeable or not. She told me if there was a leak, it will be <br />chargeable. If there wasn't a leak, it will be non-chargeable. I changed service <br />codes to non-chargeable because there was no leak. <br />08/24/2020 Sent email to Jehari and Sean Bally requested status on the vessel which was <br />taken to the shop after the incident. More specifically, we would like to know the <br />fuel disposition. <br />08/28/2020 Retrieved email sent 8-25-20 from Josh Whitfield, josh@gfmarine.com, who <br />reported that no work will be done on the vessel until he receives approval for the <br />work from responsible party's insurance. <br />09/10/2020 Received a call from Jahari Bally. He called to let me know that they are still <br />waiting for the insurance to approve the work on the boat. He also asked about the <br />bill he received for date 7-30-20. I explained that originally, I had used a billable <br />service code, however, after consulting my supervisor about billing, she told me to <br />use billable if there was a leak. If there was no leak, I was to use a non-billable <br />service code. I changed to non-billable code because from my observations on the <br />day of the incident and the morning after 7/31/20, I observed no oil residue on any <br />of the surfaces of water, boom, or asphalt where the boat was parked after <br />re-surfacing. The marina manager told me he did not feel it was a release either, <br />he was just trying to be safe in his reporting duty.
