Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COPPAQCQZ Site Location: WESTBOUND HWY 4, 3 MILES EAST OF TRACY BLVD <br />Activity Summary <br />Activity Date <br />08/04/2021 Received a spill report at approximately 7:30am regarding a diesel release into a <br />canal. I called state public health officer Liz Castro at 916-869-6627 to confirm the <br />EHD had received the report I called Sergeant Crutchfield at 209-915-6511 to <br />discuss the best route to get to the site location. The spill report was fowarded to <br />Robert McClellon and Natalia Subbotnikova. <br />I left a message for Matthew Plaza of Fish and Wildlife at 831-601-3604. I also <br />left a message with Fish and Wildlife dispatch at 916-358-1312. I received a call <br />back at 8:23am from Ryan Thiem from 415-209-4739 stating that Fish & Wildlife <br />staff will arrive on site in approximately 1 hour. <br />Inspectors Vicky yang-Lee, Elena Manzo, and Kristina Saeturn of the EHD <br />responded to the spill. When we arrived on site, Sergeant Crutchfield of the CHP, <br />Juan Armendariz of Caltrans, and Miguel Rodriguez of Mikey's Transport <br />(responsible party) were present. I observed a big rig overturned in a slough off of <br />highway 4. Diesel was observed on the road and shoulder after the truck removal. <br />I was unable to determine the extent of the release in the slough due to heavy <br />vegetation. Per Sergeant Clutchfield, he thinks approximately 80 gallons was <br />released. Sergeant Clutchfield states approximately 120 gallons was pumped <br />from the truck's diesel tank. The responsible party was on site and retained <br />Ponder for clean-up. A complaint was generated and a notice to abate was <br />emailed to miguel@mikeystransport.conn. <br />I contacted Agustin Diaz from the Ag Commissioner's office to notify him of the <br />diesel release into the slough. A map was emailed to azdiaz@sjgov.org to show <br />the location of the spill. <br />I emailed Isabella Bencomo of the Waterboard at <br />isabella.bencomo@waterboards.ca.gov to give her an update regarding the site <br />disposition. <br />I returned to the site at approximately 1:20pm. Ryan Rodriguez of Fish & Wildlife, <br />Lonnie Jones of the Coast Guard, and Jose Colin of Ponder Environmental were <br />now also on site. Clean-up was in progress by Ponder Environmental Services. A <br />hard containment boom with an absorbent liner was placed in the slough and <br />Ponder was actively removing the contaminated water hyacinths from the slough <br />and placed it into a bin. Per Jose Colin and Juan Armendarez of Caltrans, the bin <br />will be labeled and hauled to a Caltrans location in Stockton on a bill of lading. It <br />was explained to Jose that all hazardous waste should be transported with a <br />manifest. Per Sergeant Crutchfield, the clean-up company did not have the <br />appropriate equipment for the clean-up and would need to return with the <br />appropriate equipment. Clean-up is incomplete. The contaminated area was <br />secured with containment booms and cones. <br />I got a call from Warden Ryan Rodriguez of Fish & Wildlife at 3:06pm informing <br />me that the road would be closed tomorrow at approximately 9am and clean-up is <br />anticipated to start around 10am. <br />I called Isabella Bencomo at 4:29pm at 916-464-4776 and informed Isabella that <br />the clean-up is incomplete and site location has been secured. <br />Completed and printed photo log. <br />Recorded by <br />VANG-LEE <br />08/05/2021 Spill clean up site visit and assistance, onsite with K. Saetern, V. yang-Lee and F. MANZO <br />Ernest (EHD). Observed the removal of additional impacted marine vegetation. <br />Clean up was not completed today. See V.
